AGREEMENT FOR PARTICIPATION & STUDIO POLICIES

Payments may be made by cash, e-transfer or post-dated checks or credit cards online through the portal. ( checks must be dated either the 1 st or the 7th of each month) Any payments not received by the 7th will be charged a $10 late fee, If your child decides to withdraw from the program you must let us know before the 1st of the next month in order to receive a refund from the next months on. There are no refunds on registration fees or costume deposits(once the costume is ordered). Make-up classes are available pending room and ample notice. If we are forced to close due to hazardess weather, or natural disasters , or government closures ex. COVID - Refunds will not be given as these circumstances are out of our control. Zoom online classes will be offered for closures over 2 weeks.

I, ________________________ understand that dance classes may include, without limitation, dancing with props, stretching, Ballet barre work, across the floor combinations, dance tricks and dance routines in the centre, Acro and other related activities.
I, further understand that like any sport, all of the activities of the Studio’s classes involve some degree of risk of strain or bodily injury. Let’s Dance is not responsible for personal property, or injury that may occur on Let’s Dance premises, during class or while practicing. Dancers must wear the proper dance attire for each class with proper shoes. Dancers are not permitted to do any dance or acro tricks while unsupervised in the studio. It is the responsibility of the dancer/parents to ensure that these rules are understood and followed.

*If your child shows signs of illness such as fever, yellow or green discharge from nose do not bring them to class, they will be sent home to avoid spreading it to other students.

Let’s Dance Studio ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

Date: July 20,-24th 2026

Let’s Dance Studio, , their directors, officers, members, employees, volunteers, officials, participants, agents, sponsors, owners/operators of facilities and representatives (“Let’s Dance Studio”) are not responsible for any injury, damage or loss of any kind suffered by a Participant during, or as a result of, any studio activity or event, caused in any manner whatsoever including, but not limited to, the negligence of Let’s Dance Studio.


ON MY BEHALF AND ON BEHALF OF THE PARTICIPANTS, HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.


I certify that my dancer ______________________physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity.


I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.


In consideration of my application and permitting me and on behalf of my participants to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:


(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Let’s Dance Studio and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers; and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers;


(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.


I acknowledge that my dancer(’s ) may leave the location of 36 Main Street for daily excursions , I have authorized this and am aware. I also authorize that my dancer may also get into a vehicle with a teacher or volunteer of Let’s Dance Studio to get from place A to B.


I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.


The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.


Videos and photographs from the performances shall become and remain the exclusive property of Let’s Dance Studio Participants authorize Let’s Dance to use videos and photographs from the performances for promotional material, online usage, and other areas of promotion, sales, or marketing.


I, on my own behalf and on behalf of the participants, hereby warrant that I have read this agreement and fully understand its content and meaning, and sign it of my own free will and over the age of 18.
