I agree to the costume fee provided to me at registration either online or in person. I agree to pay the full costume fee by November 20, 2025. On November 20, 2025 my costume fee becomes nonrefundable due to Les Danseurs not being able to return costumes once ordered. All costumes ordered are to be picked up from the studio before recital. I agree that all costumes not picked up by recital become property of Les Danseurs.
I hereby give Les Danseurs permission to photograph and or video my child/ children in class for social media.
Upon registering I hereby agree to my set tuition price by Les Danseurs. I agree to pay my tuition by the 10th of each month, if not paid by the 10th a late fee will be added of $15. I agree to all fees which Les Danseurs provides up front upon registration. Additional fees include Shoes which are ordered through Les Danseurs, Costumes which will be added up upon registration, Recital Fee, and stocking fee.
LES DANSEURS RELEASE AND WAIVER OF LIABILITY AGREEMENT I acknowledge that I have voluntarily enrolled my child/children in dance/acrobatic/cheer classes at Les Danseurs. I am aware that these activities are athletic activities and can be hazardous or dangerous. I am aware that my child could be seriously injured by participating in these activities due to the athletic nature of dance, cheerleading and acrobatics. I am voluntarily enrolling my child/children in these activities, and acknowledge the danger involved and agree to assume any and all risks of bodily injury, death or property damage, whether those risks are known or unknown. I understand that Les Danseurs is taking precautions to prevent the spread of COVID-19 but that coming into contact with the virus is still possible. I give permission for my child to attend classes knowing that it is possible that they could come in contact with COVID-19 and will not hold Les Danseurs responsible. I hereby release Les Danseurs, staff and all instructors from any and all actions, claims or demands that I, my children, next of kin, spouse, guardians, heirs or other representatives now have or may have in the future for injury, death, or property damage related to the activities that I voluntarily enrolled my child/children in.
