I am the parent or legal guardian of the student/s that I am enrolling with 5-6-7-8 Dance Studio.

I understand that 5-6-7-8 Dance Studio may have photographs and video shots from classes, rehearsals, recitals, parades, programs, etc. that may be displayed at the studio or used on the studio website, social media or other form of advertising. I understand that the spring recital performance will be video recorded.

I give permission for 5-6-7-8 Dance Studio to use photographs and video of my child/self for business and advertising use on the website, social media and other forms of advertising for the studio.

2025-2026 School Season

I understand that there is a recital performance in the spring. I wish to have my child/self perform in the recital and I understand that I am to purchase a costume for each dance that my child/self is performing. There is a costume down-payment of $42.20 due in October. The costume balance will be due with the 4th tuition payment in March.

I understand that there is a family recital fee of $75.00 due in January. This recital fee will include a professional video recording of the spring recital and 2-recital tickets. This helps us to stream-line the video and ticket processing for our families and to cover facility costs for rehearsals and recitals.

Our school year classes run from September - May.

Tuition is billed in 4 payments (terms). Each Term is 7-8 weeks of classes. You are liable for the full tuition from the date of enrollment, unless we hear from you in writing, that you will not be attending the next term to cancel your future financial obligation. All accounts must remain current and be paid in full to be eligible to participate in class and for the annual recital showcase in May.

There will be an 18% annual finance charge on all accounts 30 days past due.

I have read and agree to the policies set forth by 5-6-7-8 Dance Studio. I understand that I am liable for tuition from the date of enrollment. I hereby forever release 5-6-7-8 Dance Studio, its employees and teachers from all liability for any and all damages and injuries suffered by me or my child while under the instruction, supervision or control of 5-6-7-8 Dance Studio.
