You must be over 18 to register for an account.

The Parent/Guardian/Adult Student (known as Client), understands and agrees to the following terms and conditions listed in this contract apply when you Register with Basically Ballet.

BEHAVIORAL:
Basically Ballet expects the following from its students:
1. Be on time for class and attend regularly. Remain home when sick with fever, diarrhea or a communicable illness in accordance with the CDC guidelines for communicable illnesses.
2. Be respectful to the Teacher(s), Staff and their classmates and do their best in class each day.
3. Attend Make Up available classes in the event of absenteeism. There shall be NO Make-Up classes in the months of May and June and when the studio deems it in the best interest of the health and safety of the students. *MC & EMC students may only Make-up classes (when offered) during the current session. MC & EMC make-up classes may be carried over if they were offered in the current session and the client re-enrolls in the directly successive session (maximum of 2 carry over Make-up classes).*
4. Come dressed in the proper and complete uniform of their class (if applicable).
5. Be an attentive & respectful listener and follow the classroom rules.
6. Accept that disciplinary action may be taken by Basically Ballet in the event that the student (youth or adult) does not strictly adhere to the rules. These actions include behavior modification techniques during class, discussion of the behavior after class, and suspension from class (for repeated unresolved behaviors) from the Basically Ballet program.
*Suspension due to disciplinary action, does not receive the opportunity of make up classes, receive a refund or release the Client from any tuition balance due to Basically Ballet.


FINANCIAL:
The Basically Ballet Dance program for which you are signing/signed up for is one of the following:
A. MC - (MINI COURSE) which ranges from 4 - 12 weeks per session.
B. EMC - (EXTENDED MINI COURSE) which ranges from 4 - 6 months
C. FYC - (FULL YEAR COURSE) which is a 31 week Leveled Course (September to June)

Tuition Cost for a FYC, EMC and MC shall be adjusted based on time of Registration. The Tuition in FULL is due upon Registration.

AUTO-PAY PLAN is available to all clients who have committed to Full Year Course classes based on the course choice and payment options available to them (see below) - Clients who choose the Automatic Payments Plan shall make their Installment Payment via credit or debit card that they will place on file. Clients will receive their 1st Invoice, which they will pay by the 5th of the prescribed month and choose the 'save card to file' option on the Invoice prior to completing the payment transaction. Subsequent payments will be applied on the 1st of each month until the tuition balance is satisfied.

A. MC (Mini Course) clients:
Mini Courses shall be all courses that are 12 weeks or less in duration per session. They shall include, but are not limited to Toddler classes, Workshops, Intensives and all other programs whose session fits within the criteria. Tuition for Mini Courses shall be paid in full at the time of Registration except in the case where other options are available with an extended commitment (i.e. Pay monthly when you commit to 5 months). Such options shall require a deposit.

Make Up classes are available and encouraged and may be taken in a comparable class during the current session only (except during times of state/national emergencies that require the long term shut down of business consisting of 2 or more months, or when the safety of the students shall be jeopardized). Then no make up classes shall be offered.

B & C. FYC and/or EMC clients:
A Registration Fee is: $35 (subject to change without prior notice) is required. A minimum of 20% of the total full course Tuition (plus the Registration Fee) shall be remitted at the time of Registration. Upon Registration, the client is automatically enrolled in the Monthly Installment Plan unless another payment option is chosen below.

* Make Up classes are available and encouraged and may be taken in a comparable class during the current year until the month of April (except during times of state/national emergencies that require the long term shut down of business consisting of 2 or more months, or when the safety of the students shall be jeopardized). There shall be no Make Up classes offered after the month March.

* FULL PAYMENT - Clients who choose the Full Payment option shall remit the balance of tuition no later than 15 days after Registration and will receive a 5% discount on the tuition cost and a $10 discount on the Registration fee.

* MONTHLY INSTALLMENT PLAN - Clients who remain in the Monthly Installment Plan (For FYC and/or EMC clients only) shall receive an Invoice each month and remit the Monthly Installment Payments by the 5th of each month until the Course Tuition has been satisfied, regardless of attendance or discontinuance from the program. Basically Ballet shall provide an additional 5 day Grace Period for monthly installment payments before charging a Late Fee. After the 10th of each month, if payment has not been remitted, the client shall be charged and pay a $25.00 Late Fee. Course Tuition, Registration and/or Payment Fees, Monthly Installment Payments and Late Fees are NON REFUNDABLE. Clients may apply in writing for a "Withdrawal" from classes with a minimum of two weeks notice of withdrawal date. An additional 1 month Tuition Installment plus $35 Withdrawal Processing fee shall be charged. No Withdrawal shall be granted after April 1st.

Dance training requires light to vigorous exercise. With all forms of exercise, there is the risk of injury. It is the desire of Basically Ballet to eliminate physical injury and/or harm. Basically Ballet will do all that is within it’s power to prevent injury by providing proper technique, limited class size and Instructor Assistants to monitor student progress and correct improper technique that could potentially result in injury. Basically Ballet will not be held liable for any injury and/or death resulting from the improper use of equipment and/or the improper execution of technique.

It is the responsibility of the Parent (or adult student) to inform Basically Ballet of any pre-existing or newly acquired condition that may be communicable and/or interfere with, hinder participation in or require attention during, the course of the training required in this dance program. The existence of a pre-existing condition, such as Asthma, Visual or Hearing Impairment, etc. may increase the risk of injury and/or illness or may simply require an adjustment in the Instructor’s teaching style. It is the responsibility of the Parent to administer and maintain all mediations. Basically Ballet does not have facilities to store medication, does not employ a Registered Nurse, nor provides staff certified in medication administration. Basically Ballet will not be held liable for injury resulting from medication administration or medication complications.

Attendance in the Basically Ballet Dance Program is very important to the success of the student. Chronic absenteeism will negatively affect the training of the student and may prohibit the student from advancing to the next level until the required skills have been met. However, the health of the student is paramount. It is Basically Ballet policy that the Client notify us and that the student remain home when sick with fever, diarrhea, vomiting or a communicable illness, in accordance with CDC guidelines for communicable illnesses. Basically Ballet shall not be held liable for any communicable illness contracted in voluntary enrollment and attendance in this program.

I have read and agree to the Basically Ballet Health Policy and will notify Basically Ballet if I (adult student) or my child is ill and will stay/keep my child home for the prescribed time according to CDC guidelines for communicable illnesses. I also will not hold Basically Ballet liable for communicable illness that may be contracted in voluntary enrollment and attendance, nor hold Basically Ballet liable for injury and/or death as a result of participation in this program.

Basically Ballet believe that pictures and/or videos of our students are the best examples of the great training and wonderful atmosphere offered at Basically Ballet Studio. Basically Ballet will use photographs and video of our students in current and future instruction to promote the excellent training offered at Basically Ballet Studio. It may also use, with consent, these forms of media for public promotion for the purposes it sees fit and in accordance with laws pertaining to minors. The video and still pictures are the property of Basically Ballet and shall be used in all media forms. Basically Ballet will do it’s best to conceal/mask the identity of any student who specifically requests anonymity in the public promotion of Basically Ballet. The uses of the photos and/or videos include but are not limited to still photography and video displayed within the studio, newspaper, billboard, television, internet, cinema and/or any other means of public exposure. The images (past, current & future) once taken and/or delivered to Basically Ballet and/or an agent thereof, are the property of Basically Ballet to be used and/or distributed as it sees fit for the purposes of instruction and promotion of the studio and it’s services.
