A CREDIT CARD NUMBER IS REQUIRED TO REGISTER (regardless of preferred payment method).

(Based on season fees broken into 8 month payments)
Withdrawal prior to Sept 1st: Refund of full season, minus annual registration fee
Withdrawal after Sept 1st before first class: Refund of Oct-Apr fees
Withdrawal prior to Oct 1st: Refund of Nov-April fees
Withdrawal prior to Nov 1st: Refund of Dec-April fees
Withdrawal prior to Dec 1st: Refund of Jan-April fees
Withdrawal prior to Jan 1st: Refund of Feb-April fees.
- No refund will be issued after January 1st, monthly fees will be required due to time to re-space and rework choreography and routines.
- Costume, competition and examination fees are all non-refundable, including cases of illness and/or injury, as per competition rules.
- Class fees refunded due to illness or injury will be dealt with on a case by case basis.

Liability Waiver
Physical activities such as dance, acro, drama and musical theater can carry a risk of injury to participants. I hereby release Leader School of Dance and its employees of any liability whatsoever arising out of any injury, damage or loss which may be sustained by myself, my minor children, or any other family members, during involvement with Leader School of Dance. This includes activities held in any studio, on the property at the studio location during lessons, classes, performances, or rehearsals and during any other activities sponsored by Leader School of Dance.

Medical Release
In the event of an emergency, I give permission for Leader School of Dance staff to authorize necessary medical treatment, by a licensed physician for my son or daughter, without personal liability, if I am unable to be contacted in a timely matter.

Photo/Video Release
We are asking permission to be allowed to take pictures and/or videos of your children to show the community the success of our programs. Your right to privacy is taken very seriously and as such, we will use dancer's names upon parent approval as well as limit any personal information being shared. These photos and videos may be used for newspaper, newsletter, display boards, website, and other public information. I relinquish all rights of any photos or videos to be used by Leader School of Dance. By signing this release, I hereby state that I am 18 years of age or older and am the parent/legal guardian of a minor child. I hereby give permission for photographs and/or video to be taken of my children for the promotion of Leader School of Dance and its programs.
