I/we (the dancer and the parent/guardian) understand that participation in dance classes at The Hive Dance Company may involve the risk of injury. Despite the precautions taken by the studio and its staff, accidents and injuries can occur. By signing this release form, I/we (the dancer and the parent/guardian) assume all risks associated with participating in classes and using The Hive Dance Company’s facilities.
The studio reserves the right to combine or cancel classes as a result of low registration. I understand that the studio reserves the right to cancel or reschedule classes at any time and for any reason. In the event of a class cancellation, I understand that I will not be entitled to a refund or make-up class.
I understand that participation in classes and events at The Hive Dance Company may involve exposure to contagious illnesses. By attending, I acknowledge and accept that The Hive Dance Company cannot guarantee a completely risk-free environment. I voluntarily assume all risks related to the exposure of any contagious illness for myself and my child(ren) and release The Hive Dance Company, its staff, and representatives from any liability related to such exposure.
I give permission for The Hive Dance Company to photograph and videotape my son/daughter for publicity, advertising, educational and archival purposes, and by my participation, I agree to waive all claims to these materials.
I understand that as parent or guardian; will be contacted if medical attention is required during class time. If I cannot be reached, I hereby authorize an administrator/instructor of The Hive Dance Company to arrange for treatment as necessary, and I will be responsible for any charges incurred.
I consent to my child using physical touch with other students during class for partnering in choreography.
SPRING TERM STUDENTS: Each recreational class will perform a routine in our recital at the end of Spring Term. Each class will require a costume. Costume fees are an additional charge of $130 + HST per class and is due upon registration. All accounts must be paid in full before a student can participate in the recital. Tickets for the recital are not included in tuition. You must tell us upon Spring Registration if your child will not be participating in the recital.
All class fees are non-refundable. A credit will be given for absence due to medical condition and/or injury (a doctor's note is required). Missed classes are non-refundable and no make-up classes are offered. Costumes fees are non-refundable. Registration fees are non-refundable. Refunds for summer camps, less a cancellation fee of $75.00 per camp, will be given upon written notice of cancellation prior to May 31. No refunds will be processed after this date. There are no discounts for days not attended.
I agree to release The Hive Dance Company from responsibility including all teachers, dancers, staff members and facilities from any cause of action, claims or damages now and in the future. I will not hold The Hive Dance Company liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or any other type of injury/disease/medical condition and also any personal property damage/loss which may occur on the premises before, during or after classes/performances.
I consent to the teacher using physical touch during class for correction/spotting.
