PARENT:
I, the parent/guardian of the registered dancer/s, have read the Jazzle Dance Studio Handbook and Jazzle Dance Company handbook and understand the requirements for my dancer to participate in Jazzle Dance Studio dance program. I fully comply with all rules and regulations within Jazzle Dance Studio Code of Conduct. I also understand the disciplinary ramifications if any part of this agreement is broken including (but not limited to) dismissal from the studio. I am aware that dance is a team effort will make every effort to do my share of assisting at the studio on behalf of my child. I understand I am required to be present for parent meetings or have someone present on my behalf.


DANCER:
As a dancer at Jazzle Dance Studio, I have been read the studio Handbook/ Code of Conduct and Jazzle Dance Company handbook and understand what is expected of me to remain a dancer in good standing at the studio. I will keep myself knowledgeable and informed of all rules and understand that the consequences of poor behavior or inappropriate actions include dismissal from the studio. I will do all I can to uphold Jazzle Dance Studio's mission and respect other dancers, the studio, directors, staff, instructors and volunteers at all times.


I certify that I have read and acknowledge Jazzle Dance Studio's handbook, media policy and code of conduct. By registering for classes I agree with all policies.

TUITION POLICY
Tuition charges begin on the date of class registration. Families are responsible for all tuition fees until a written 30-day drop notice is submitted via email, regardless of class attendance. Same-day or week-of drop notices will not be accepted.

Please note that all payments are non-refundable and no credits will be given.

Dancers who are inactive for 30 consecutive days will be automatically withdrawn from the program; however, tuition will remain due through the official drop date. Accounts delinquent for 60 days or more will be referred to collections.

AUTOPAY POLICY
All payments must be submitted electronically through our Studio-Pro Parent Portal. Enrollment in autopay with a valid credit or debit card on file is required. Tuition is automatically debited on the 1st of each month. If the payment is declined, a second attempt will be made on the 15th. If the second attempt fails, a $30 late fee will be applied. Dancers with an outstanding balance beyond 30 days will not be permitted to attend class or participate in studio events.

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM for Jazzle Dance Studio

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

I certify that I am in good health and that I do not pose a health risk to the public.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event. In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I HEREBY WAIVE, RELEASE, AND DISCHARGE Jazzle Dance Studio and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, private lessons, or any activity I may participate.

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Jazzle Dance Studio and their employees, volunteers, or the activity or event holders, activity or event sponsors, activity or event volunteers.

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise. The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old)
The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor, the parents, of the legal guardian.

Any photographs and/or videos that are taken by or in conjunction with Jazzle Dance Studio and corresponding activities may be used for any and all purposes.

Never post anything that could compromise the self-esteem of dancers who attend Jazzle Dance Studio.

Never post any negative comments about Jazzle Dance Studio, its instructors/staff/volunteers, performances, or events. Likewise, never post any negative comments about a fellow dancer or peer.

If you post videos of class or rehearsal, do not post choreography in its entirety.

No Jazzle/Jazzle Elite rehearsal footage should be posted on any social media platforms
