I understand that Intricate Dance Chicago has safety protocols and rules in place to keep myself/ my child safe. I do not hold Intricate Dance Chicago LLC, its staff, or dancers responsible for any injury that occurs to myself or my child.
I understand Intricate Dance Chicago has rules and precautions in place in order to maintain safety. I do not hold Intricate Dance Chicago LLC for any personal property that becomes damaged, lost, or stolen.
I will report anything suspicious or unsafe immediately to a staff member.
I understand that tuition is due each month, and after seven days, if my payment is not complete, I will be charged a late fee.
I recognize that photo, video, voice recording, and another similarity could be used for performance, production, marketing, social media, commercials, and more, and allow Intricate Dance Chicago to have all rights to use a photo, video, and voice of myself/my child, without receiving privacy rights or compensation for use.
I understand there will be other children in my child’s class, and I will be aware of others around me, and maintain respect and follow studio policies.

Dance Studio Injury Waiver and Liability Policy
At Intricate Dance Chicago, the safety and well-being of all students are very important to us. Dance, like any physical activity, involves inherent risks of injury. By participating in classes, rehearsals, performances, or any activities by the studio, students and/or their parents or legal guardians acknowledge and agree to the following:
1. Assumption of Risk
Participants understand that dance activities may involve physical exertion, movement, and use of equipment that carry the risk of injury. These risks may include, but are not limited to, muscle strains, sprains, falls, or more serious injuries. Participation is voluntary, and all participants assume full responsibility for any risks involved.
2. Health and Fitness Responsibility
It is the responsibility of the participant or guardian to inform the studio of any medical conditions, injuries, or limitations prior to participation.
3. Release of Liability
Intricate Dance Chicago, its owners, instructors, staff, and affiliates shall not be held liable for any injuries, accidents, or damages that may occur during or as a result of participation in studio activities. By enrolling, participants waive any and all claims against the studio for personal injury or loss.
4. Responsibility for Personal Belongings
The studio is not responsible for lost, stolen, or damaged personal items brought onto the premises.
5. Compliance with Instructions
Participants agree to follow all safety instructions, rules, and guidance provided by instructors. Failure to do so may increase the risk of injury and may result in removal from class.
6. Emergency Medical Care
In the event of an emergency, the studio may seek medical treatment on behalf of the participant. Any associated costs are the responsibility of the participant or their guardian.
7. Acknowledgment and Agreement
By registering for classes or participating in any studio activities, participants (or their parent/legal guardian) acknowledge that they have read, understood, and agreed to this policy.

I allow Intricate Dance Chicago to use my own or my child's photo/voice/video for print, television, marketing videos, social media, commercials, or any other use related to Intricate Dance Chicago. I wave my rights to privacy and other legal rights pertaining to such use of myself or my child. I understand that there is to be received no fee for use of these materials such as photo, video, or voice.

Please follow dress code as directed and enter the classroom dressed in the proper attire. If arriving more than 15 minutes late, students may be asked to sit and wait until there in an appropriate time to join with the rest of the class. We ask that gym shoes that are worn for Hip Hop are not to be worn outside in order to protect our studio. Hair must be pulled out of your face and off your neck. No excessive jewelry. No gum or food in the studio. No cell phones. No inappropriate language. Be respectful, be a role model, and maintain a positive attitude at all times!
Attendance
If you are not able to make it to class, please contact the studio as soon as possible. You are able to schedule a makeup class in the same class level you currently taking for a day that works best for you. Please note that classes are not refundable and we will not hold credit due to absences. The desk manager will advise you if you have any questions.
Late Tuition
Tuition is non-refundable and non-transferable. We also accept cash and check. We will charge a 15% late fee of your balance if payments are overdue. If not paid, we cannot allow your student in class until your account is up to date. Recital costumes will be held until the dancers balance is cleared. Balances must be cleared by recital in order to participate. We do require a card to be kept on file. Dancers must enroll in auto pay to add the card on file. We will send out an email asking who would like to be removed from the auto pay for that month, that way, if you do not prefer to pay on the first day of the billing period, you have two weeks to do so. 10 days after those two weeks, a 15% charge will be added to your balance that is due, and the card on file will be charged automatically.
