Dance Studio Injury Waiver and Liability Policy
At Intricate Dance Chicago, the safety and well-being of all students are very important to us. Dance, like any physical activity, involves inherent risks of injury. By participating in classes, rehearsals, performances, or any activities by the studio, students and/or their parents or legal guardians acknowledge and agree to the following:
1. Assumption of Risk
Participants understand that dance activities may involve physical exertion, movement, and use of equipment that carry the risk of injury. These risks may include, but are not limited to, muscle strains, sprains, falls, or more serious injuries. Participation is voluntary, and all participants assume full responsibility for any risks involved.
2. Health and Fitness Responsibility
It is the responsibility of the participant or guardian to inform the studio of any medical conditions, injuries, or limitations prior to participation.
3. Release of Liability
Intricate Dance Chicago, its owners, instructors, staff, and affiliates shall not be held liable for any injuries, accidents, or damages that may occur during or as a result of participation in studio activities. By enrolling, participants waive any and all claims against the studio for personal injury or loss.
4. Responsibility for Personal Belongings
The studio is not responsible for lost, stolen, or damaged personal items brought onto the premises.
5. Compliance with Instructions
Participants agree to follow all safety instructions, rules, and guidance provided by instructors. Failure to do so may increase the risk of injury and may result in removal from class.
6. Emergency Medical Care
In the event of an emergency, the studio may seek medical treatment on behalf of the participant. Any associated costs are the responsibility of the participant or their guardian.
7. Acknowledgment and Agreement
By registering for classes or participating in any studio activities, participants (or their parent/legal guardian) acknowledge that they have read, understood, and agreed to this policy.
