I consent to my child's participation in The Meredith Gravely School of Dance physical programs, classes, activities and performances. As the parent/guardian of my enrolled student, I acknowledge that I am aware of the significant risks and possibility of physical injury associated with participation in such programs, classes, activities and performances. Enrolled students and their parents/guardians recognize and willingly assume such risks. Further, by registering for any program, class, activity or performance at The Meredith Gravely School of Dance, parents/guardians represent their child does not have any illness, disability or impairment that will endanger their child or others in connection with their child's participation in the dance programs, classes, activities and performances offered by The Meredith Gravely School of Dance
I, the parent/guardian and my child/student, enter this agreement under the express condition that The Meredith Gravely School of Dance, its owners, agents, employees, independent contractors, volunteers, vendors and other associated people shall be free from all liabilities and claims for damages and/or suits for or by reason of any injury, including death, or damage to or loss of property of our child/student from any cause or causes whatsoever while participating in The Meredith Gravely School of Dance programs, classes, activities or performances, either at the studio or in a public venue while representing the School.
Parents/guardians and students understand and acknowledge that The Meredith Gravely School of Dance is taking precautionary safety measures and following the Centers for Disease Control and Prevention guidelines as it relates to COVID-19 to provide a healthy environment for dance classes and dance related activities. I acknowledge my child is healthy and will take personal responsibility in following the outlined COVID-19 policies and procedures while participating in dance and dance related activities at The Meredith Gravely School of Dance.
Parents/guardians authorize The Meredith Gravely School of Dance, through its staff and personnel, to obtain, consent to and authorize emergency medical care or treatment in the event such care is necessary or appropriate in the opinion of the staff and personnel for the benefit of a student caused by accident, injury or illness. The Meredith Gravely School of Dance will only exercise such consent and authorization for medical care or treatment in the absence of a student's parent/guardian and only if it is unable to reach a student's parent/guardian within a reasonable period of time given the circumstances of the emergency. Parents/guardians will be responsible for making payment of all costs of any such medial care or treatment directly to the hospital, physician or facility providing such care.
I agree to pay for the entire season, regardless of later withdrawal, in full and no later than April 1, 2025. The tuition contract will become null and void only in the event that the studio closes. I agree to pay a $30 return check fee per returned check and a $30 service charge per month for any tuition not paid by April 1, 2025. Furthermore, if the school seeks legal action to collect any part of the tuition, service charges and fees, I agree to pay all legal fees and collection costs the school incurs.
I agree to allow my child to be photographed and/or videotaped and that such images may be published in an outlet used to promote or publicize The Meredith Gravely School of Dance, Inc.
