Liability disclaimer: I understand that dancing and dance classes constitute social/athletic/ physical activities and that injuries may occur. I am willing and able to accept full responsibility for my child's safety and well-being. I understand that good dance training involves touching and adjustment of the student's body by the instructor. Acknowledging this, I hereby release and agree to hold harmless Lisa Graziano, Teachers and LHG Creative from any and all claims and liabilities, which may arise out of my child’s participation.

Policy Agreement: It is understood that the student is enrolled for the entire session; no deductions or refunds will be made for absence or withdrawal, voluntary or involuntary. It is also understood that classes may transition to a virtual platform at any time if needed for health and safety purposes. No refunds or credits will be provided for this transition.

I understand that performing arts classes are physical and that injuries may occur. I understand there are inherent risks associated with participation in classes, rehearsals and performances, and I release LHG Creative and all its employees, volunteers, and related parties from any and all claims or liability arising directly or indirectly from this participation.

I understand that good dance training involves touching and adjustment of the student's body by the instructor. Acknowledging this, I nearby release and agree to hold harmless Lisa Graziano, LHG Creative and their teachers from any and all claims and liabilities, which may arise out of my child's participation.

I agree to indemnify LHG Creative for any costs, expenses, or liability arising out of my child’s participation, including the cost of any medical care that may be given to my child or any expenses or fees incurred by LHG Creative, or liabilities arising from any lawsuit resulting from any damage or injuries caused by my child in the course of his or her participation in the activity.

In case of an emergency involving my child, I understand that every reasonable effort will be made to contact me. In the event I cannot be reached, I hereby give my permission to LHG Creative staff to secure proper treatment, including hospitalization, for my child. Medical care providers are authorized to disclose to the adult in charge of the child all examination findings, test results, and treatments provided for purposes of the child’s medical evaluation, follow up, and communication with the child’s parents or guardian, and/or determination of the child’s ability to continue in the program activities.

I understand that LHG is a commitment based school and agree to the schools no refund policy. LHG Creative does not offer refunds of any kind. It is understood that when registered for a class, students are enrolled for the entire semester. It is understood that should an emergency or public health issue occur, classes at anytime are subject to be transitioned onto a virtual platform. No deductions, refunds or credits will be made for any reason, voluntary or involuntary. By registering I am in agreement with these terms.

I have reviewed LHG's website and agree with its mission and policies. I understand that LHG Creative is a professional school. While not all students are expected to want to become professionals, I agree to uphold the community and it's classes with respect. I agree to treat LHG's policies, dress code, classes, teachers and community with professionalism and respect. I understand that participation at LHG Creative is voluntary and requires students to abide by applicable safety rules and standards of conduct.

I understand that LHG Creative uses photographs of their students for advertising and I give permission to LHG Creative to use photographs of my child in their advertising, on social media and on the website www.lhgcreative.org Students are not identified in photographs. If there are safety reasons that my child should not be photographed, I agree to notify LHG Creative by emailing TheSchool.LHGCreative@gmail.com to let them know prior to my Childs participation in class.

LHG does not permit bullying, discrimination or negative talk about the school, its teachers, its staff, programing, fellow parents or students at any time. I understand that an inability to follow these rules will result in our immediate removal from the school.

LHG Creative classes have uniforms. I understand LHG Teaches Professionalism. This means students are required to come properly prepared to class (LHG provided uniform, hair done properly and on time). We understand mistakes happen and sometimes you may have an off day. However, I understand that if a student is consistently unprepared or late for class, the teacher may ask that student to sit out that day. Credits, make ups and refunds are not provided should a child be asked to sit out.
