Liability Waiver & Assumption of Risk Agreement
I, the undersigned parent/guardian (or participant if 18 years of age or older), understand that participation in cheerleading, tumbling, stunting, jumping, conditioning, competitions, camps, clinics, and other athletic activities involves inherent risks, including but not limited to serious injury, permanent disability, paralysis, and death.
I voluntarily choose to allow myself or my child to participate in activities offered by Powerhouse Cheer and knowingly assume all risks associated with participation.
I certify that I/my child is physically able to participate in these activities and have not been advised otherwise by a physician. I understand that it is my responsibility to inform Powerhouse Cheer of any medical conditions, injuries, or limitations that may affect participation.
In consideration of being allowed to participate in Powerhouse Cheer activities, I hereby release, waive, discharge, and hold harmless Powerhouse Cheer, its owners, directors, coaches, employees, volunteers, independent contractors, facility owners, and representatives from any and all claims, demands, actions, damages, losses, expenses, or liabilities arising from or related to participation in any Powerhouse Cheer activity, including those resulting from ordinary negligence.
I understand that emergency medical treatment may be sought if deemed necessary and that I am responsible for any resulting medical expenses.
I grant permission to Powerhouse Cheer to use photographs and videos of myself or my child for promotional, marketing, social media, website, and advertising purposes without compensation.
I have carefully read this Liability Waiver & Assumption of Risk Agreement, fully understand its contents, and voluntarily agree to its terms.
Athlete Code of Conduct Agreement
At Powerhouse Cheer, we are committed to creating a positive, safe, and respectful environment where athletes can learn, grow, and succeed. By participating in Powerhouse Cheer, athletes agree to follow the expectations outlined below.
Athlete Expectations
I agree to:
Show respect to coaches, teammates, parents, officials, and competitors at all times.
Display good sportsmanship whether winning or losing.
Follow all instructions given by coaches and staff.
Arrive on time and prepared for practices, competitions, events, and team activities.
Maintain a positive attitude and encourage my teammates.
Use appropriate language and behavior at all times.
Treat all equipment, facilities, uniforms, and personal property with care.
Practice safely and avoid actions that may put myself or others at risk.
Represent Powerhouse Cheer in a positive manner both in person and online.
Unacceptable Behavior
The following behaviors will not be tolerated:
Bullying, harassment, intimidation, or discrimination of any kind.
Fighting, aggressive behavior, or threats toward others.
Excessive disrespect toward coaches, staff, teammates, parents, or officials.
Inappropriate language, gestures, or behavior.
Social media posts that negatively impact Powerhouse Cheer, its athletes, families, coaches, or reputation.
Deliberate disruption of practices, competitions, or team activities.
Theft, vandalism, or intentional damage to property.
Disciplinary Action
Failure to follow this Code of Conduct may result in disciplinary action, including but not limited to:
Verbal warning
Parent meeting
Temporary suspension from team activities
Removal from specific events or competitions
Dismissal from the program
Powerhouse Cheer reserves the right to determine appropriate disciplinary action based on the severity and frequency of any violation.
Acknowledgment
I understand that participation in Powerhouse Cheer is a privilege and that I am expected to uphold the values of teamwork, respect, responsibility, and sportsmanship. I agree to follow this Athlete Code of Conduct and understand that failure to do so may result in disciplinary action, including dismissal from the program without refund.
Medical Consent & Emergency Treatment Authorization
I, the undersigned parent/legal guardian of the athlete listed below (or participant if 18 years of age or older), authorize Powerhouse Cheer and its representatives to obtain emergency medical treatment for the athlete if an injury, illness, or medical emergency occurs and I cannot be reached immediately.
I understand that reasonable efforts will be made to contact the parent/guardian or emergency contact before treatment is administered when circumstances permit.
I authorize licensed physicians, emergency medical personnel, hospitals, clinics, and other healthcare providers to provide any medical care deemed necessary for the health and safety of the athlete, including emergency transportation, examination, treatment, medication, diagnostic procedures, and hospitalization.
I understand and agree that I am solely responsible for all medical expenses incurred as a result of any injury, illness, or medical treatment received by the athlete.
I certify that the athlete is physically capable of participating in cheerleading activities and that all known medical conditions, allergies, medications, and special needs have been disclosed.
I understand that participation in cheerleading and athletic activities involves the risk of injury. I authorize emergency medical treatment as described above and agree to hold harmless Powerhouse Cheer, its owners, coaches, employees, volunteers, and representatives from liability related to emergency medical care obtained on behalf of the athlete.
I have read and understand this Medical Consent & Emergency Treatment Authorization and voluntarily agree to its terms.
Photo & Video Release Agreement
I, the undersigned parent/legal guardian of the athlete listed below (or participant if 18 years of age or older), grant permission to Powerhouse Cheer to photograph, record, and use images, video recordings, audio recordings, and likenesses of myself and/or my child for promotional, educational, marketing, and informational purposes.
I understand that photographs and videos may be used in various forms of media, including but not limited to:
Social media platforms
Website content
Advertising and marketing materials
Promotional videos
Flyers, banners, and brochures
Newsletters and email communications
Print and digital publications
I understand that these photographs and recordings may be edited, reproduced, published, distributed, or displayed by Powerhouse Cheer without further notice.
I acknowledge that no compensation, royalties, or other payment will be provided in exchange for the use of these photographs, videos, or recordings.
I release and hold harmless Powerhouse Cheer, its owners, coaches, employees, volunteers, and representatives from any claims, demands, or liabilities arising from the use of such photographs, videos, recordings, or likenesses.
This authorization is voluntary and will remain in effect for the duration of the athlete's participation with Powerhouse Cheer unless revoked in writing. Any revocation will apply only to future use and will not affect materials already published or distributed.
