Xpressions For Performing Arts – Waiver & Agreement

MEDIA RELEASE
I grant permission to Xpressions For Performing Arts to photograph and/or video participants for use in promotional materials including social media, website, advertising, and archival purposes. I understand that no compensation will be provided.

ASSUMPTION OF RISK & LIABILITY WAIVER
I understand that participation in dance, acrobatics, and performing arts activities involves inherent risks including, but not limited to, physical injury, falls, strains, or illness.

I voluntarily assume all risks associated with participation.

I hereby release, waive, and discharge Xpressions For Performing Arts, its owners, directors, instructors, employees, agents, and affiliates from any and all claims or liabilities, including those arising from negligence, related to participation in any classes, rehearsals, performances, competitions, or related activities.

MEDICAL AUTHORIZATION
I certify that the participant is physically able to participate.

In the event of injury or emergency, I authorize Xpressions For Performing Arts staff to administer first aid and/or seek medical attention if necessary. I understand that I am financially responsible for any medical treatment provided.

ENROLLMENT & PAYMENT POLICY

Tuition and fees are non-refundable and non-transferable
No prorations, refunds, or credits will be given for missed classes
All payments must be made on time to maintain enrollment

PARTICIPATION POLICY
Studio staff, instructors, and their family members may participate in classes, performances, and competitions but are not eligible for certain awards or prizes.

Competition judges are independent and are not affiliated with or influenced by Xpressions For Performing Arts.

CONDUCT & SAFETY
Xpressions For Performing Arts reserves the right to dismiss any student or parent for disruptive, unsafe, or inappropriate behavior without refund.

The studio is not responsible for students before or after scheduled class times. Parents/guardians are responsible for timely drop-off and pick-up.

INDEMNIFICATION
I agree to indemnify and hold harmless Xpressions For Performing Arts from any claims brought by or on behalf of the participant or any third party arising from participation.
