I hereby give my child permission dance at Illume Dance Company. I waive the right to any legal action against Illume Dance Company or its teachers for any injury sustained on studio property or at any Illume Dance Company event. I understand that I am enrolling my dancer in a program of physical activity with inherent risks of injury and have informed the teacher/directors of any physical, social, or mental limitations that would help them safely and effectively teach my child. Further, I acknowledge that because of the physical nature of dance there may be appropriate physical contact between the teacher and students and occasionally between students. I understand that physical contact is required and necessary for proper dance instruction.

I give permission for Illume Dance Company to take and use photos/videos of my child during class to share with myself and other parents (via text, email, or social media) and/or for promotional or educational material.
