I understand that all enrolled families at Le Rose Dance Academy, Inc., must have an up-to-date credit card on file. Tuition will be due on the 1st of each month in which you can pay credit, cash, or check. If it is not paid by the 4th of the month, then your credit card on file will be ran on the first business day after the 4th of the month. Credit card information will be kept in a locked file. If payment is declined on provided credit card, your dancer will not be able to attend class until payment is paid in full. I authorize Le Rose Dance Academy, Inc. to use the entered account information for all my account billing for the current season, and all following seasons until the dancer drops or I opt out of AutoPay. I am allowing Le Rose Dance Academy, Inc. to clear all invoices due to my account on this credit card or debit card (which include registration fees, tuition, costumes, late fees, convention and competition fees, recital fees, rental of space, events, and any other studio or dance related items purchased on my account). I take full responsibility for payment of all charges and understand that a $10 late fee will be applied to my account if my credit card or debut card is declined and cannot by processed for the amount due at the time. This agreement will remain in effect until I notify Le Rose Dance Academy, Inc. of my intent to discontinue the use of my credit/debit card. All accounts must be current to participate in monthly classes and annual recital. I understand registration fees, costume deposits, late fees, and balances along with recital fees are non-refundable.

I represent that my dancer(s) is physically, medically, and mentally able to participate safely in such activities as those at Le Rose Dance Academy, Inc. I agree to assume all risks associated with my dancer's participation in all activities at Le Rose Dance Academy, Inc. or any facility used for activities associated with Le Rose Dance Academy, Inc. I hereby waive, release, and discharge Le Rose Dance Academy, Inc., its owner, staff, contracted help, or volunteers of all present or future claims and liabilities of any kind whether sickness, injury to body, damage to property, or other loss arising from my dancer's participation in activities offered by Le Rose Dance Academy, Inc. including, but not limited to, dance instruction, cheer instruction, tumbling instruction, instruction of any style class offered, private lessons, rental of Le Rose Dance Academy, Inc. for a class, private event, such as birthday parties/private gatherings and, rehearsals and performances on or off Le Rose Dance Academy, Inc. property. I also agree that I am responsible for delivery and pickup of my dancer to and from Le Rose Dance Academy, Inc. activities on the property or at an alternate facility/property and that Le Rose Dance Academy, Inc. is not responsible for a dancer once they have walked out of the door of the facility or completed an activity or event at an alternate facility/property.

During rectial week, I agree to follow all policies and rules at our rehearsals, recitals, and events that take place each day during that week. All recital location rules and policies are to be followed. A few examples include, no smoking on campus, no food or drink the auditorium, and any social distancing rules or guidelines. I also agree to the release of liability at performance locations.

By signing, I agree that I have read this statement of release in its entirety, understand it, and herby agree to its contents and terms.

During the season, dress rehearsals, recitals, and events, Le Rose Dance Academy, Inc. takes photographs and recordings of their classes, students, events, and performances. These will be used for, but not limited to, advertisements and promotional purposes. By signing, I agree that my dancer can be photographed and recorded, and that their images can be used, but not limited to, the following: Facebook, Instagram, newspaper articles, DVDs, USBs of performances, sign, displays at Le Rose Dance Academy, Inc. or an event being produced by or for Le Rose Dance Academy, Inc. Dancer's names will not appear on any used media materials. I also agree that I will not, at any time, present or future, be compensated for the use of these images or recordings. I understand that this a photography/recording release to Le Rose Dance Academy, Inc. of images and recordings for the dancers listed under my account.
