Injury/Health Liability

By registering your child, you confirm that they are in good health and will undergo daily health screening procedures. If your child displays symptoms such as coughing, fever, nasal congestion, or shortness of breath, you agree to notify Luna P.L.A.Y Kids immediately and keep your child home. You understand that participation in physical activities involves inherent risks of injury or illness, including COVID-19, and you agree to release Luna P.L.A.Y Kids, its owners, and staff from all liability for injuries, illness, or other damages that may occur during activities.

Photo Release

This waiver applies to all Luna PLAY classes. Please note that for promotional purposes, pictures and videos may be taken of your child and used for marketing on social media, our website, and other platforms. If you do not agree to this, please visit your main dashboard, select the drop-down menu labeled "More," then click on "Waiver & Policies" to opt-out.
