Injury/Health Liability

Applies to all classes with Luna Play Kids.
By signing this waiver, I confirm that my child is in good health and will undergo a daily health screening. If my child exhibits symptoms such as nasal congestion, cough, loss of taste or smell, runny nose, shortness of breath, headache, or fever (unrelated to pre-existing conditions like asthma or allergies), I will notify Tracy Frimpong (owner, Luna P.L.A.Y Kids) immediately and refrain from sending my child to class.

I understand that participation in in-person activities carries risks, and I assume all responsibility for the decision to send my child. I release Luna P.L.A.Y Kids, its owner, and staff from all liability related to any illness contracted (including COVID-19) or injuries sustained during any activities or programs.

This agreement releases Luna P.L.A.Y Kids from all liability for injuries that may occur during activities, regardless of negligence. I acknowledge the inherent risks of physical activities, including minor and major injuries. By signing below, I waive all rights to legal action against Luna P.L.A.Y Kids and agree to the terms and conditions outlined.
