I acknowledge and consent to allow Tri-Cities Dance Company to use: photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio’s website, or on social media sites (such as the studio’s facebook or instagram account). I understand that in some cases, my child and/or my name may be used. I further understand no financial compensation will be for use of these photos/videos. By signing electronically, I agree that I have read, understand, and agree to this agreement.

Tuition is due on the 1st of every month. A credit card on file is required to register for classes. If the payment is not made by the 15th, unless you have contacted us directly and made a payment arrangement, your account will be charged a late fee and your card on file will be automatically run on the 16th of the month.

If you choose to drop a class, the studio must be notified in person or via email regarding this dropped class. If not, auto-pay will continue to be run and refunds will not be issued.

I have chosen to have my child participate in various class instruction given by Tri-Cities Dance Company. I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Tri-Cities Dance Company is not in any way responsible for making such a determination. In consideration of my child’s enrollment in any dance instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Tri-Cities Dance Company from all claims, costs, liabilities, expenses or judgments, including attorneys’ fees and court costs for any occurrences in connection with any dance instruction. I assume all risks to my child in connection with any instruction and further release Tri-Cities Dance Company and its owners and employees from liability for any injury sustained by my child while he or she is enrolled in any dance instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen. This includes any exposure to the COVID-19 virus or other illnesses. By electronically signing this agreement, I have read, understand, and agree to this information.
