At Dedication Dance Co. (“DDC”), we are committed to providing a safe and supportive environment for all dancers. However, as with any physical activity, participation in dance classes and related activities involves a risk of injury. Please read the following carefully and sign below to acknowledge your understanding and agreement.
WAIVER AND RELEASE OF LIABILITY
By signing this agreement, I acknowledge and agree to the following:
Assumption of Risk
I understand that participation in dance classes, rehearsals, performances, camps, and any related activities at or affiliated with Dedication Dance Co. involves physical activity that may result in injury. These risks include, but are not limited to, slips, falls, muscle strains, sprains, and other injuries. I voluntarily assume all such risks associated with my (or my child’s) participation.
Release of Liability
I hereby release, waive, discharge, and hold harmless Dedication Dance Co., its owners, instructors, staff, volunteers, and affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may occur during participation in DDC programs or while on the premises, whether caused by negligence or otherwise.
Medical Emergencies
In the event of a medical emergency, I authorize DDC staff to provide basic first aid and, if necessary, seek emergency medical treatment at the nearest medical facility. I understand that I am responsible for all medical expenses incurred.
Photo and Video Release
I grant permission for Dedication Dance Co. to take and use photographs and/or videos of myself or my child for promotional and educational purposes, including but not limited to websites, social media, and printed materials. No names will be published without additional consent.
Compliance with Studio Policies
I agree to follow all rules and policies set forth by Dedication Dance Co. I understand that failure to comply may result in dismissal from class or program participation.
Tuition Policies Acknowledgment
I confirm that I have read and understand Dedication Dance Co.'s tuition policies, including payment due dates, late fees, withdrawal procedures, and refund policies. I acknowledge that continued enrollment is contingent upon timely tuition payment and adherence to these financial terms.
Acknowledgment and Signature
I have read and fully understand this liability waiver and release. I am signing this agreement freely and voluntarily and intend for it to be a complete and unconditional release of all liability to the greatest extent allowed by law.
