The practice of dance involves the risk of physical injury (with bruises being the most likely injury and broken bones or other more serious physical injuries also being possible.) Understanding this I declare:
1- That I am willing to accept resposibility for such an eventuality.
2- That I have adequate medical insurance for such an eventuality.
3- That I will not hold any persons associated with Full Force Dance Academy liable for any injury that the dancer(s) might sustain while practicing dance.
4- I agree to pay any fees associated with dance in a timely manner or pay any associated late fees.
5- I agree to give a 30 day notice if not returning to the dance studio. If no notice is given, any fees associated with the dance season will be charged to my account.
6- I agree that I am over 18 and have the rights to make decisions for the student registering.
Further, I understand the physical demand of this activity and the practice required for its development and mastery. As a consideration for the dancer's safety and enjoyment, as well as that of other students, i commit to dedicate necessary practice of the instructions and techniques given during class.

Monthly tuition is due on the first of every month. A late fee will be applied to your account on the third of the month if payment is not received.

Liability Waiver and Release

On behalf of myself and my children, I understand that there are risks associated with, arising out of and inherent to the activities provided through Full Force Dance Academy LLC. They include, but are not limited to, illness, injury, and death. In recognition of this acknowledged risk, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Full Force Dance Academy, studio owner, employees, instructors, (hereinafter collectively referred to as “Full Force Dance Academy”.)
I hereby agree to release Full Force Dance Academy and hold Full Force Dance Academy harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks arising out of active participation in classes, performances, and activities through Full Force Dance Academy on behalf of myself, the legal guardian, and the participant/s.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the associated risks. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
The participants for whom I am registering have my permission to participate in all Full Force Dance Academy classes, performances, and activities. I warrant the information is complete and correct and that I do have the authority to act and speak on behalf of the participants I enroll in classes at Full Force Dance Academy. I further release Full Force Dance Academy of all liabilities associated with my children’s attendance at Full Force Dance Academy.

Consent for photography/filming

I give consent to the Full Force Dance Academy to use photographs and/or video recordings, including images of me both internally and externally, to promote the studio and display images publicly. These images could be used in print and digital media formats including print publications, websites, e-marketing, posters, banners, advertising, film, social media, teaching and research purposes.

I understand that images on websites can be viewed throughout the world and not just in the United States and that some overseas countries may not provide the same level of protection to the rights of individuals as the United States.

I understand that some images or recordings may be kept permanently once they are published and be kept as an archive of Full Force Dance Academy.

I have read and understand the conditions and consent to my images being used as described.

Students and parents may not photograph or record any material in class without the teacher’s permission.

The personal data collected on this form will be held securely and will only be used for administrative purposes.

You have the right to request to see a copy of the information we hold about you and to request corrections or deletions of the information that is no longer required. You can ask Full Force Dance Academy to stop using your images at any time, in which case it will not be used in future advertising but may continue to appear if already published.
