I understand that participating in dance and/or acrobatics requires physical activities that carry a certain risk. I do hereby agree to hold harmless Flying Feet LLC, Inspire Dance Studio, its Director, or Employees from any and all claims for personal injuries to myself, and/or child before, during or after participating in said activities. I understand that there are no credits or refunds for missed classes due to religious, school or health events for the exception of long term medical issue that prevents participation for longer than 1 month (a doctor's note will need to be provided). I understand that I am responsible for payment for all services within the given time frame and payment holds my/my child's place in class, failure to do so will terminate services until balances are paid in full.

Unless I choose to Opt-out, I understand that my child's image may be used in video or picture for marketing purposes only.

The information you provide is used only by Inspire Dance Studio administration. It is never shared or sold to anyone. Inspire Dance Studio (Flying Feet LLC) uses the information as a means of contact, class registration, updating accounts, and when given permission, run credit card payments.
