I am applying for and/or have applied for membership of Port Macquarie Gymnastics Club Inc. I agree to be bound by and accept the following “Terms and Conditions and Parent/Guardian consent and declaration†TERMS AND CONDITIONS AND PARENT/GUARDIAN CONSENT AND DECLARATION 1. There are inherent risk associated with the activities which may result in personal injury (even of a serious nature) to a participant. I fully accept and agree to bear these risks. I understand that participation in gymnastics activities carries with it a reasonable assumption of risk. 2. I confirm that the information given in the medical history section of the Parent Portal on behalf of the participant is true and correct to the best of my knowledge. 3. In case of emergency, I hereby give consent to a certified First Aid Officer to administer emergency First Aid to the participant and, to obtain any medical or ambulance assistance as required. I agree to pay costs incurred. 4. I understand that the yearly registration fee must be paid in full prior to the participant being allowed to attend any classes in the gymnasium (even holiday fun days). This is a mandatory requirement outlined by our insurance underwriters and by Gymnastics NSW. 5. I understand that Port Macquarie Gymnastics has a NO refund policy. Term fees must be paid in full prior to confirmation of enrolment. 6. I understand that where the participant cannot attend classes due prolonged illness or injury, and a Doctors Certificate is provided, Port Macquarie Gymnastics will credit the remaining fees to the following Term's fees. Should the participant not return, NO refund will be given. 7. I understand that it’s my responsibility as parent/guardian to to ensure that all class bookings are made in a timely manner and within the Club's deadlines to guarantee the position and class of the participant. I have read and understand this membership application and class enrolment and agree to the terms, conditions, consents and declarations as required and stated herein. I confirm the information provided in this application is complete and correct to the best of my knowledge and I undertake to advise the Club promptly of any changes that may occur. I agree that Port Macquarie Gymnastics Club and its management, parents committee, coaches, volunteers and staff shall be released from and, shall not incur any responsibility or liability whatsoever for any accident or injury to the participant or, any damage to or loss of property of the participant. In that regard, I agree to indemnify Port Macquarie Gymnastics Club against any such liability.
