By registering for classes, participating in our programs, and providing personal information to us, you consent to the practices outlined in this Privacy Policy.

Personal Identifiers: Name, address, phone number, email address, emergency contact information

Payment Information: Credit/debit card details, billing address, and payment history

Dance Program Information: Enrollment details, attendance, competition participation, and performance records

Contractual Information: Details from competition team contracts, AutoPay agreements, and any other signed agreements

2. How We Use Your Information

Providing Services: To enroll you and your dancer in our programs, schedule classes, manage payments, and communicate important updates.

Billing and Payments: To process monthly tuition charges, costume fees, competition fees, and other charges, as outlined in your AutoPay Agreement.

Competition Fees: As stated in your competition contract, we may charge additional competition-related fees, which will be handled separately. You will receive advance notice of these charges via a competition fee schedule.

Communication: To send newsletters, updates, and any relevant information related to your dancer’s participation in our programs.

Legal Compliance: To comply with legal obligations, enforce our terms of service, resolve disputes, and address any issues related to your enrollment or contract.

3. Sharing Your Information
We may share your personal information with third parties in the following situations:

Service Providers: We may share your information with trusted third-party providers to help us deliver services (e.g., payment processors, communication tools, website hosting). These service providers are required to use your data only for the services they perform on our behalf and must protect your information.

Legal Compliance: If required by law or legal process (e.g., subpoena or court order), we may disclose your information to comply with such requests.

Collection Agencies: If an account is 90 days past due and the balance exceeds $250, we may refer your account to a third-party collection agency, as per the terms outlined in the AutoPay Agreement and Competition Contract.

As a resident of California, you have certain rights regarding your personal information under the California Consumer Privacy Act (CCPA):

Right to Know: You may request details about the personal information we collect, use, and share.

Right to Delete: You have the right to request the deletion of your personal information (subject to certain exceptions).

Right to Opt-Out: If applicable, you have the right to opt-out of the sale of your personal information (please note that we do not sell your personal data).

Right to Non-Discrimination: You will not face discrimination for exercising your rights under the CCPA.

Notice: By signing the AutoPay Agreement, competition contracts, or any related documentation, you acknowledge that you have read, understood, and consented to this Privacy Policy and the associated terms. Adagio Dance Company

By signing below, I authorize Adagio Dance Company to automatically charge the payment method I have provided for all tuition, costume fees, and other program-related charges associated with my dancer’s enrollment.

Terms of Authorization
Monthly tuition will be charged automatically on the 1st of each month.

Competition fees will be charged separately according to a published competition fee schedule, which I will receive in advance.

I understand that this authorization will remain in effect until the end of the current dance season or until I provide written notice of cancellation, in accordance with Adagio’s drop and withdrawal policy.

I may not remove or delete the payment method on file through my DanceStudio-Pro (DSP) account. To update or remove my card, I must email info@adagiodance.org.

I agree to provide updated payment information promptly if my card is lost, expired, or replaced.

Failed Payments & Late Fees
If a payment is declined, I understand that a $20 late fee will be applied after the 5th of the month, and an additional $25 late fee will apply if the balance remains unpaid after the 10th of the month.

If my account becomes 90 days past due and the balance owed is over $250, I understand that my account may be referred to a third-party collection agency, and I will be responsible for any additional collection and legal fees, as permitted under California law.

My Rights & Acknowledgment
I acknowledge that this recurring payment authorization complies with California Civil Code and state consumer billing laws, including proper notice and consent.

I agree not to dispute valid charges processed under this agreement, and I accept responsibility for any balances due.

I understand that participation in AutoPay is required for enrollment at Adagio Dance Company.

Adagio Dance Company LLC
We (both dancer and parent/guardian) realize that participation in dance classes and activities involve an inherent risk for possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, We (both dancer and parent/guardian) assume all associated risks of personal injury to the use of any and all spaces used by Adagio Dance Company LLC.
We (both dancer and parent/guardian) agree to release and hold harmless Adagio Dance Company LLC including its teachers, dancers, staff members, and facilities used by Adagio Dance Company LLC from any cause of action, claims, or demands now and in the future. Adagio Dance Company LLC will not be held liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, We (both dancer and parent/guardian) agree to obey the class and facility rules and take full responsibility for our behavior in addition to any damage caused to the facilities utilized by Adagio Dance Company LLC.
I understand that Adagio Dance Company LLC is licensed, accredited and insured. In the event that anyone observe any unsafe conduct or unsafe conditions before, during or after classes, it will be reported to either the Executive Director, Individual Instructor, or available staff member as soon as possible.

Authorization:
I hereby authorize the designated school personnel to administer the medication specified above to my child as instructed. I understand and agree to the following terms and conditions:
-I certify that the information provided on this form is accurate and complete to the best of my knowledge.
-I understand that it is my responsibility to provide the medication in its original container with the original prescription label affixed, including the student's name, medication name, dosage, and prescribing physician's name.
-I agree to inform the school promptly of any changes to the medication or its administration instructions.
-I release and discharge the school, its employees, agents, and representatives from any and all claims, demands, or causes of action arising out of or in connection with the administration of the medication specified above, except for claims arising from negligence or willful misconduct.

Emergency Procedures:
In the event of an emergency related to the administration of the medication specified above, the school is authorized to take appropriate action, including contacting emergency medical services and/or the student's healthcare provider.
This authorization shall remain in effect until revoked in writing by the undersigned parent/guardian.

Photo and Video Release Waiver

By checking the box below and submitting this form, I, the undersigned parent/legal guardian, give my consent to Adagio Dance Company to photograph and/or record video footage of my child during classes, rehearsals, performances, and other studio events. I understand that the images or video recordings may be used for promotional, educational, or marketing purposes.

Purpose of Use
The photographs and videos may be used for the following purposes:

Marketing and promotional materials (e.g., flyers, posters, social media, website)

Studio educational content

Public displays, newsletters, and digital publications related to the studio

Acknowledgment and Release
By consenting to this release, I understand and agree that:

These images and videos may be shared publicly on digital and social media platforms, and may be accessible worldwide.

There will be no compensation for the use of my child’s image, name, or likeness.

I waive any right to inspect or approve the final product or use of the materials.

Right to Withdraw
I understand that I may withdraw my consent at any time by notifying Adagio Dance Company in writing via email at info@adagiodance.org. However, withdrawal of consent will not affect materials already published or distributed.
