By enrolling in and participating in the IHeart Dance Studio training program, including classes, rehearsals, performances, competitions, conventions, travel, and all related activities, I acknowledge and agree to the following:
Assumption of Risk & Release of Liability
I fully understand that participation in dance involves inherent risks, including but not limited to sprains, pulled muscles, broken bones, collisions, and contagious illnesses. Injuries may be minimal, serious, or catastrophic.
I voluntarily assume all risks associated with participation and hereby forever release, waive, and hold harmless IHeart Dance Studio, its owners, directors, staff, instructors, independent contractors, choreographers, vendors, venues, and affiliates from any and all claims, damages, liabilities, losses, or causes of action of any kind, including negligence or future negligence, arising from participation in any IHDS activity, including travel to and from events.
Competition Team & Program Acknowledgment
I understand that participation in IHDS training programs and competition teams may require mandatory rehearsals, extended hours, travel, strict attendance, and adherence to all studio rules, schedules, and codes of conduct.
Medical Authorization
I affirm that the participant is physically able to participate and has no condition that would limit safe participation. I authorize IHDS to obtain emergency medical treatment if necessary and accept full financial responsibility for such care.
Personal Property
I understand that IHDS is not responsible for lost, damaged, or stolen personal property at the studio or at off-site events.
Photo, Video & Likeness Release
Participants (including dancers, parents, guests, teachers, and choreographers) authorize IHeart Dance Studio to photograph, video, or otherwise capture their likeness during any IHDS engagement and grant permission for such media to be used for promotional, marketing, educational, and archival purposes without compensation.
Intellectual Property & Content Use
I understand that all choreography, routines, music edits, videos, and original content created or owned by IHeart Dance Studio are the intellectual property of the studio. Reproduction, sharing, or posting of such content on social media or other platforms is not permitted without written permission from the Studio Director.
Financial Responsibility
Primary account holders acknowledge responsibility for all fees agreed upon during registration and understand that IHDS may pursue legal action to collect unpaid balances in accordance with studio policies.
Agreement to Studio Policies
By completing online registration, I confirm that I have read, understood, reviewed, and agree to abide by all IHDS studio policies, as outlined in the studio handbook and registration materials.
Protection of Property
I understand and agree that it is my child’s and my sole responsibility to safeguard our personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release IHeart Dance Studio, its affiliate dance studios, event sponsors, employees and dance teachers from all liability for loss or damage to our personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by IHeart Dance Studio.
Medical Attention
In case of physical injury or medical emergency, I hereby authorize IHeart Dance Studio to make necessary arrangements to transport my child or myself to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In an extreme emergency, or if my child is under 18 years of age, I understand that IHeart Dance Studio will attempt to notify the person(s) I have named below as my emergency contact(s) of my condition and how to reach me.
Photo Release
IHeart Dance Studio reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting IHeart Dance Studio and its programs. Students, or parents of students who are minors, who do not wish to comply with this policy must notify IHeart Dance Studio prior to participation in class.
Acknowledgement of Waiver
In signing this Release, I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, satisfactory, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my child’s or myself participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to my child or myself.
Acknowledgement of Policies
In signing this Release, I furthermore acknowledge that I have received a copy of the IHeart Dance Studio Policies and agree to adhere to all guidelines.
