General Liability
I/we realize that participation in dance classes and activities may result in personal injury. Despite precautions, accidents and injuries may occur. By signing this release form I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Foundations Dance Academy Incorporated, including virtual classes hosted at home or an out of studio setting.
I/we agree to release and hold harmless Foundations Dance Academy Incorporated, including its teachers, dancers, staff members, and facilities used by Foundations Dance Academy Incorporated from any cause of action, claims, or demands now and in the future. I/we will not hold Foundations Dance Academy Incorporated liable for any personal injury or any personal property damage, which may occur on or off the premises before, during, or after classes. This includes virtual class instruction as well. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by Foundations Dance Academy Incorporated.
I understand that Foundations Dance Academy Incorporated is licensed, accredited, and insured. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Director, an instructor, or staff member as soon as possible.
Policies & Procedures
I understand that Foundations Dance Academy Incorporated, its staff, and Director will not be held responsible for lost or stolen items in the classroom or on the premises at any time. I have also read and understood the Brochure, which outlines the dress code, policies and procedures.
Promotional Activities
Designated Foundations Dance Academy Incorporated staff may capture photos or videos of student progress throughout the year. Content may then be used for promotional purposes and for keepsakes in the annual studio yearbook.
I have read the above and understand that to decline promotional activities I (as a parent) am required to check the "Do not post public photos of this student" box on my students Studio Pro summary page.
I authorize Foundations Dance Academy to utilize my student’s name and/or pictures/videos for the annual studio yearbook. This yearbook is a studio copy only - not for sale or publication. If you wish to decline, please send an email to info@foundationsdanceacademy.com.
Teacher Technique Adjustments / Spotting
I/we realize that participation in dance classes and activities may involve physical contact for spotting purposes and technical adjustments. By signing this release form I/we assume all risks related to the use of spotting techniques and technique adjustments by instructors.
I/we agree to release and hold harmless Foundations Dance Academy Incorporated, including its teachers, dancers, staff members and facilities used by Foundations Dance Academy Incorporated from any cause of action, claims, or demands now and in the future. I/we will not hold Foundations Dance Academy Incorporated liable for any personal injury, illness, or any personal property damage, which may occur on the premises before, during or after classes. I/we also agree to obey the class and facility rules and take full responsibility for my/our behaviour and any damage I/we may cause to the facilities utilized by Foundations Dance Academy Incorporated.
