Electronic Signature Agreement.
By selecting "I agree," I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

I understand that this policy ONLY APPLIES to students enrolling as a regular, weekly student and DOES NOT APPLY to students enrolling for drop-in classes, pop-up classes, workshops, intensives, or auditions. I further understand that, if I/my child is enrolling as a regular, weekly student, I/my child will be continuously enrolled at West Hawaiʻi Dance Theatre until I provide notification that we would like to withdraw. I understand that tuition is billed every four (4) weeks on the first day of the 4-week class period and will be automatically charged to my payment card. If I do not have a payment card on file or my payment card has expired, I agree to make payment by the first day of the 4-week class period. If I/my dancer wants to withdraw, I will notify West Hawaii Dance Theatre by emailing staff@whdt.org or calling/texting 702-509-1352 at least three (3) days prior to the next billing. Refunds are not given for classes not attended during a period or for early withdrawal.

I recognize the risks of illness or injury inherent in any dance and movement exercise program. Participation in classes offered by West Hawaiʻi Dance Theatre is upon the express agreement and understanding that I am waiving and releasing West Hawaii Dance Theatre from any and all claims, costs, liabilities, expenses, and judgments, including attorney's fees and court cost (herein collectively "claims") arising out of my (or my child's) participation in these classes. I hereby further agree to indemnify and hold harmless West Hawaiʻi Dance Theatre (dba West Hawaii Dance Academy; dba West Hawaiʻi Island GYROTONIC®) its staff and contractors, board of directors, The Queen Lili'uokalani Trust, and Colliers International Real Estate Management Services from and against any and all such claims. In the event of an emergency, I give my permission for the emergency contact on file to be called, in addition to a doctor or ambulance.

I hereby grant West Hawaiʻi Dance Theatre and associated choreographers permission to use photos or videos of me and/or my child on their websites, social media pages, advertisements or other publicity purposes for the studio. I understand and agree that I will not be compensated for the use of and waive my right to inspect or approve any photos or video in which I or my child may appear. I understand and agree that any photos or videos taken by West Hawaiʻi Dance Theatre or vendors they contract are their property. I hereby hold harmless, release, and forever discharge West Hawaiʻi Dance Theatre, its staff and contractors, and board of directors, from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. If I want to opt-out of this authorization, I will submit my request in writing to West Hawaiʻi Dance Theatre by emailing staff@whdt.org.
