1. Acknowledgment of Risk

I, the undersigned participant and/or parent or legal guardian, understand that participation in dance classes, lessons, rehearsals, performances, competitions, workshops, camps, events, activities, and other programs or activities offered by Krylo Dance Studios LLC (“Krylo Dance Studios”) involves inherent risks.

These risks may include, but are not limited to, falls, collisions with other participants or objects, slips and trips, muscle strains, sprains, fractures, dislocations, physical exhaustion, injuries resulting from improper technique or equipment, injuries resulting from the actions of other participants, and other physical or personal injuries. I understand that dance involves strenuous physical activity and that injuries may occur even when appropriate instruction, supervision, and safety precautions are provided.

I voluntarily choose to participate, or permit my child(ren) to participate, in activities at or associated with Krylo Dance Studios with full knowledge of these risks.

2. Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release and agree not to hold liable Krylo Dance Studios LLC, its owners, officers, employees, instructors, independent contractors, volunteers, agents, representatives, successors, and assigns from claims, demands, causes of action, damages, or expenses arising out of or related to participation in Krylo Dance Studios activities, including claims for personal injury, illness, property damage, or loss.

This release applies to activities occurring:

At the Krylo Dance Studios facility;
At theaters, auditoriums, schools, competition venues, and other off-site locations;
During classes, rehearsals, performances, recitals, competitions, workshops, camps, demonstrations, and special events;
During studio-sponsored transportation or travel, if applicable; and
During any other activity, event, or program organized, sponsored, or participated in by Krylo Dance Studios.

This release is intended to apply to the fullest extent permitted under the laws of the State of Rhode Island.

3. Assumption of Responsibility

I understand that I am voluntarily participating, or permitting my child(ren) to participate, in dance and related activities. I accept responsibility for evaluating whether participation is appropriate based on the participant's physical condition, abilities, and limitations.

I agree to notify Krylo Dance Studios and its instructors of any condition, limitation, injury, or other circumstance that may affect the participant's ability to safely participate.

4. Medical Treatment Authorization

In the event that I or my child becomes injured or ill while participating in a Krylo Dance Studios activity and I cannot be immediately reached, I authorize Krylo Dance Studios, its employees, instructors, agents, or representatives to seek or arrange appropriate emergency medical assistance on my behalf or on behalf of my child.

I understand that Krylo Dance Studios is not responsible for the cost of medical treatment, transportation, emergency services, or other medical expenses incurred as a result of an injury or illness.

I understand that Krylo Dance Studios personnel are not medical professionals and that any first aid or assistance provided is not a substitute for professional medical care.

5. Minors

If any participant listed on this form is under eighteen (18) years of age, I represent that I am the participant's parent or legal guardian and have the authority to enter into this agreement on the participant's behalf.

I understand that I am agreeing to the terms of this waiver and release with respect to the minor participant(s) listed above, to the extent permitted by Rhode Island law.

6. Personal Property

I understand that Krylo Dance Studios is not responsible for lost, stolen, or damaged personal property brought onto studio premises or to studio-related activities, except to the extent otherwise required by law.

7. Compliance With Studio Policies

I agree that all participants will follow reasonable safety instructions, studio rules, instructor directions, facility rules, and applicable policies established by Krylo Dance Studios.

I understand that Krylo Dance Studios reserves the right to restrict or discontinue a participant's participation when, in the studio's reasonable judgment, the participant's behavior or condition presents a safety concern to themselves or others.

8. Acknowledgment and Voluntary Agreement

I have carefully read this Liability Waiver, Release & Assumption of Risk. I understand that I am giving up certain legal rights by signing this agreement.

I have had the opportunity to ask questions regarding this agreement and understand its terms. I acknowledge that my participation, or my child's participation, in Krylo Dance Studios activities is voluntary.

I understand that this agreement is intended to be legally binding upon me and, to the extent permitted by law, my heirs, personal representatives, and assigns.

9. Severability

If any provision of this agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect to the fullest extent permitted by law.

10. Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the State of Rhode Island, without regard to conflict-of-law principles.
