I as a Parent/Guardian in consideration of my child/charge being allowed to participate in the classes at The Barrie School of Dance Inc. , do hereby release and agree to hold harmless and indemnify The Barrie School of Dance Inc. , their Director, consultants and employees from any and all claims, actions, damages, costs, expenses without any limitation whatsoever, whether consisting of loss, personal injury or property damage that may occur as a result of participation . I acknowledge that my child is in good health and physically fit to participate in the classes offered. I grant consent to The Barrie School of Dance Inc. to take photographs of my child/charge for the purpose of promoting The Barrie School of Dance Inc. Freedom of Information and Protection of Individual Privacy Act I further understand that the personal information on this form is collected by The Barrie School of Dance Inc. for administrative purposes only. I have read and understood this agreement and agree to the terms and conditions.
