Illness/Injury I acknowledge and agree that in participating at Studio 531, there is a possibility of illness and/or injury to the dancer/student. I will not hold Studio 531, its Owner/Director, or Instructors responsible for any illness or injury that may occur while participating in Studio 531. Photo Acknowledgement I hereby authorize Studio 531 to publish photographs taken of my child(ren) and my child(ren)'s name, for use in Studio 531's printed publications and websites. I acknowledge that since my participation in publications and websites produced by Studio 531 is voluntary, I will receive no financial compensation. I further agree that my child(ren)'s participation in any publication and website produced by Studio 531 confers upon me no rights of ownership whatsoever. I release Studio 531, MLR Dance II, LLC, its contractors, and its employees from liability for any claims by me or any third party in connection with my child(ren)'s participation. Financial Responsibility By registering your child into classes at Studio 531 you have entered into an agreement with MLR Dance II, LLC, dba Studio 531. You agree to pay tuition for classes by quarter due dates as established by Studio 531. Tuition payments that are late shall be charged a $15 late fee. Any account that is 30 days past due shall be charged 29.00% interest on any unpaid balance monthly. Any account that has an outstanding balance at the beginning of the next dance season shall not be eligible to enroll students until such time as any outstanding balances are paid in full. *In the unforeseen event that your child is no longer able to participate at Studio 531 a written notice must be given to the Director. Charges applied to your account up to the date that notice is received are the responsibility of the account holder.
