DANCE & TUMBLING RELEASE FORM: The Dance Realm Studios will not be held liable for any personal injury that may be encountered during dance or tumbling programs or events.
LATE TUITION PAYMENTS Tuition payments received past the 15th day of the tuition months will result in a $10.00 late fee accruable per week past due. (For example: 5 weeks late = $50.00 late fee.) The system will automatically bill the late fee each week.
I hereby give Sue Beiting and/or The Dance Realm permission to publish photographs and/or videos of my child(ren) listed below on the The Dance Realm website, Facebook, Instagram, and/or any other social media or print media. I represent and warrant that I am the parent or legal guardian of the listed minor child(ren) and I have the full legal capacity to execute this release.
No refunds on tuition or costumes will be granted.
I hereby release The Dance Realm of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of classes, camps, private instruction, choreography or any activity my child participates in. I certify that I and my children have read and will comply with The Dance Realm's new procedures and fully understand its content. I am aware that this is a release of liability and a contract and I am signing it of my own free will. Procedures The following procedures offer a healthy dance environment for your child while at The Dance Realm: 1. Students must have a completed, signed Release of Liability Waiver submitted on the first day of class. 2. Please encourage your student to use bathroom facilities before class. If necessary, a teacher will assist your student in the restroom wearing gloves and a mask while doing so.
I understand that there is required dancewear for my child's class (September through May). The studio sells the required dancewear (at no mark up) on the following dates only: Fort Thomas Thursday, July 31st – 6-8pm Wednesday, August 20th– 6-8pm Beechmont Wednesday, July 30th – 6-8pm Thursday, August 21st– 6-8pm TAP SHOES WILL BE AS FOLLOWS THIS YEAR: All 45-minute, 60-minute, and 75-minute classes will continue with a black Bloch Merry Jane tap shoe. 90-minute classes and Teams 1 and 2 will be going into a black Capezio Downtown tap shoe. THIS A NEW SHOE FOR TDR. It has a stronger sole that will enable our students to do more advanced tapping and also has a better sound quality. Older, more experienced dancers (2-hour classes and Teams 3, 4, 5, and 6, with the exception of Fort Thomas Team 6 and Fort Thomas Thursday 3:30-5:30) will be going into a black Capezio Cadence tap shoe. THIS IS A NEW SHOE FOR TDR. It has a stronger sole that will enable our students to do more advanced tapping and also has a better sound quality. We are no longer using Bloch tap flex tap shoes. HOWEVER, team and regular students will be permitted to use theirs if they have a black pair that still fits. IMPORTANT: We will also fit team dancers in the month of December/January for any dancer that will be needing the Downtown or Cadence tap shoes due to their shoes currently fitting now. ALL TAP SHOES WILL BE BLACK THIS YEAR. TDR WILL NO LONGER BE USING A TAN COLORED TAP SHOE.
RETURNED CHECKS ALL RETURNED ˆCHECKS WILL ˆRESULT ˆIN A $35.00 RETURN ˆCHECK ˆFEE.
Studio class tuition is only accepted on the trimester schedule. Tuition may not be paid monthly. Payments are due the day of registration, the first week in November and February. Costume fees for our annual recital are due the first week of November. No refunds on tuition or costumes will be granted.
