1. Acknowledgment of Risks
I, the undersigned parent/guardian, acknowledge that participation in dance classes and related activities involves physical activity and carries inherent risks of injury. I understand that accidents can happen and injuries may occur despite all safety precautions.
2. Release of Liability
I agree to release, waive, and hold harmless Dance Infinity, its owners, instructors, staff, and agents from any liability for injuries, damages, or losses that may occur during participation in dance classes, rehearsals, performances, or related activities, whether caused by negligence or otherwise.
Please note: Dance Infinity is not responsible for injuries or accidents that occur during participation.
3. Medical Treatment & Emergency Contact
In the event of injury or illness, I authorize Dance Infinity staff to seek emergency medical treatment for my child. I agree to be responsible for all medical expenses incurred.

4. Payment Policy
I agree to pay all tuition and fees by the specified deadlines. I understand that dancers will not be permitted to participate in classes or performances if tuition or fees are unpaid or overdue. Failure to make timely payments may result in suspension or dismissal from the studio until all balances are cleared.
5. Photo and Video Release
I grant permission for photographs or videos of my child taken during studio activities to be used for promotional purposes, including social media, website, and advertising.
6. Studio Policies & Conduct
I acknowledge that my child will adhere to the studio's rules and code of conduct. Violations may result in dismissal from the studio.
7. Acknowledgment and Agreement
By signing below, I confirm that I have read this waiver and understand its contents. I also acknowledge that further details regarding policies, procedures, and rules are included in the studio’s 2025-2026 Policy document available on the studio’s website.
