By electronically signing this waiver, I acknowledge and agree to the following terms and conditions as a requirement for participation in any dance classes, recitals, competitions, and related activities offered by Fusion Dance Academy, whether held on or off studio premises. I understand and acknowledge that participation in this program involves inherent risks of physical injury, and certify that I am physically fit to participate in this program. I voluntarily agree to assume full responsibility for all such risks, whether foreseen or unforeseen, and regardless of the cause, on behalf of myself or my child.
I hereby waive, release, and discharge Fusion Dance Academy, its owners, instructors, employees, agents, contractors, affiliates, volunteers, successors, and assigns (collectively referred to as “Fusion Dance Academy”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, illness, personal injury, or death that may be sustained as a result of participation in Fusion Dance Academy programs, regardless of whether caused by the negligence of the studio or otherwise. I understand and agree that Fusion Dance Academy is not responsible for any lost, stolen, or damaged personal property, including but not limited to phones, wallets, jewelry, shoes, costumes, dance bags, or other belongings brought to or left at the studio.
I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY. I FULLY UNDERSTAND ITS TERMS AND UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING MY RIGHT TO SUE. I SIGN THIS DOCUMENT FREELY AND VOLUNTARILY.
