I acknowledge that participation in dance classes, rehearsals, performances, and other activities offered by Charade Dance Academy involves inherent risks, including injury, illness, or death. I certify that I have disclosed all relevant health information about myself/my child(ren) and will update the studio with any changes. In consideration of participation, I, on behalf of myself and my heirs or representatives, release and waive any and all claims against Charade Dance Academy, its owners, staff, volunteers, contractors, affiliates, landlords, and insurers for any injury, loss, or damage, including death, arising from participation in studio activities.

I authorize Charade Dance Academy staff to administer basic first aid to me and/or my child(ren) in case of injury or illness. I understand Charade Dance Academy staff are not trained medical professionals but will provide care to the best of their ability. I authorize Charade Dance Academy staff to seek emergency medical treatment and transportation for me and/or my child(ren) if they deem necessary. I permit myself/my child to be treated by emergency personnel and hospital staff. I agree to cover all costs related to medical treatment, transportation, and care.

Tuition is calculated based on a 4-week month and must be paid through the Dance Studio Pro Autopay function with a credit card. Your credit card will be charged on the 1st of each month. If your card is declined or the full balance cannot be processed, you must provide a new payment method. If not updated by the 3rd of the month, a 25% late fee will be added to the outstanding balance.

Tuition payments are NON-REFUNDABLE & NON-TRANSFERABLE.

To drop any classes, email charadedance@gmail.com by the 20th of the month prior to the next payment date. Failure to notify us by this deadline will result in non-refundable tuition processing for the next month. Phone or verbal notifications are appreciated, but you will be requested to follow up with a written notice via email.

Students are encouraged to attend any and all classes for which they are enrolled. However, if a student is showing signs or symptoms of illness, please have them stay home from class to avoid physical stress and contagion. Dancers who come to class and exhibit signs of sickness may be asked to remove themselves from the class.

Make-ups may be requested via email for absences due to illness or injury. In the case of a non-illness absence, please request make-ups via email at least 2 days before the absence. Make-up classes must be scheduled and taken within 30 days of the absence. Absences cannot be credited to the next month.

Make-up classes are offered as a courtesy and may not always be granted due to availability. We encourage dancers to attend all scheduled classes and plan accordingly. Make-up classes are final and cannot be rescheduled. Missing a make-up class results in forfeiture.

Students should come to class prepared with the correct dance uniform, shoes, and a water bottle.

Once class is in session, parents and siblings not enrolled in the class must stay in the reception area and avoid crowding doorways. Parents and siblings should not enter the dance floor to avoid distracting the dancers.

Parents and guardians of younger dancers are requested to stay for the duration of class to assist with restroom breaks if needed. Younger dancers must be potty-trained before enrolling in class. No diapers!

Dancers must be picked up promptly after class. Dancers are not permitted to leave the studio building until a parent or guardian arrives. If needed, you may request that a staff member escort your dancer to the parking lot.

Each class requires a different uniform. Please purchase the correct uniform for your dancer to wear for each class. Correct uniform, hair, and shoes must be worn to class each day to help instructors inspect dancers’ alignment and posture as well as promote professionalism and discipline.

Uniform items must be Charade Dance Academy official dancewear and can be purchased at the front desk. Please ask a staff member at the front desk to clarify the correct hair, uniform, and shoes that must be worn for your dancer’s class. No jewelry or watches are allowed, except for small post earrings.

Dancers without the correct and/or approved uniform may be denied class participation. Any tuition and/or fees paid for the class will be forfeited.

I allow Charade Dance Academy to take photos and videos of my dancers and family members for public and studio use.

I will not take personal photos or videos of my child or other dancers during class unless invited to do so.
