TUITION
-Registration fee: DUE at time of enrollment
-Our full season runs from August - May.
-Tuition is monthly, so if your dancer decides dance is not for them, you are only required to pay for the months they are enrolled. To withdraw, you must notify us via EMAIL ONLY by the 15th of the month, or you will be charged for the next monthly tuition. Verbal notification of withdrawal will not be accepted.
-Monthly payment is not related to the number of classes in the month. Class days allowing for holidays, inclement weather and occasional instructor illness are built into our yearly calendar and will not be prorated. Short months (2-3 weeks) will be charged the same tuition as long months (4-5 weeks).
-Tuition must be paid in full before the 5th of every month. Payments received after the 5th will incur a $10 late fee
-Tuition is paid by autopay
-Returned Check Fee $25
-All past tuition and registration fees are NON-REFUNDABLE.

COSTUME FEE
Yearly Costume Fee: $100/student. (50% will be charged with your December Tuition, and the remainder will be charged with your March Tuition). If you enroll after December, the full amount will be due with your March Tuition.

AUTOPAY
Monthly payment is by autocharge through debit/credit or ACH. There is a convenience fee of 3.5% per transaction. You may choose to pay for the year in full at a 10% discount. *Late Fees & NSF Fees: There will be a $10.00 late fee applied for payments received after the 10th of the month. There is a $20.00 charge for any checks, credit cards, or ACH transactions not processed due to NSF.

INCLEMENT WEATHER:
For the safety of our dancers, if we have to close for inclement weather, we will confirm our closing via email around 3pm that same day. We typically wait to decide UNTIL after 2pm

DRESS CODE: Students are required to abide by all studio dress codes, posted on our website.

PHOTOGRAPHY RELEASE
I do hereby grant permission to the Dance Co-op to photograph my child or children. If photographed or filmed, my child's image may possibly be reproduced for the Dance Co-ops promotional media at any time in any form, including but not limited to print, digital, and electronic publishing via the Internet, without compensation. My child will not be identified if his or her image is used for promotional purposes.

GENERAL RULES: Please discuss these rules with your dancers! Thank you!
-No glass bottles and only water with a screw-on lid or sports bottle.
-No peanuts, pets, or smoking.
-Please let us know if your dancer will be absent from class due to illness or other obligations.
-If your student is ill, please do not send them to class.
-Make-up classes are available other nights of the week and can be made up anytime during the current season.
-No cell phones, smart watches , or earbuds are allowed in the class rooms at any time. If you choose to bring them to class, please put them in the designated area in the hallway..
-All children must be accompanied by a parent/guardian at all times unless they are in class..
-Please take your dancer to the restroom BEFORE class begins to avoid accidents and disruptions to the class.
-Excessive tardiness in picking up your child will result in additional fees.
-No parents, friends, or siblings are allowed in the classroom at any time without explicit permission from the instructor
-Please label ALL shoes, bags, etc!

WAIVER OF LIABILITY
THE DANCE AND ART CO-OP LLC does not carry medical insurance for its students. Although all activities are performed with the students' safety as the utmost concern, all students are required to be covered under their own family's insurance policies. If an injury should occur, you will only be compensated through your own family insurance policy.

I understand that there is a risk of injury to my child with the participation in any dance class or rehearsal. I certify my child is in good health and able to participate fully in physical activity such as dance. I understand that I am waiving my right to bring legal action and to assert any claims against THE DANCE AND ART CO-OP LLC , its respective agents or employees, for any and all negligence, loss, liability, damage, or personal injury, including death, that may be sustained by the participant, while in or upon the premises or any premises under the control of THE DANCE AND ART CO-OP LLC , its owners or agents, or en route to or from any said premises.

MEDICAL RELEASE
I do hereby grant permission to THE DANCE AND ART CO-OP LLC, and their respective agents and employees to seek medical aid and hospital services as they deem necessary for the child noted on this form in the event he/she should sustain injury or illness while attending classes and rehearsals. I realize that every effort will be made to contact me first in the event of a medical emergency involving my child, and agree to indemnify and hold harmless THE DANCE AND ART CO-OP LLC agents in seeking medical care for my child. I have also indicated on this form my child's doctor and his/her contact information, a non-parent emergency contact, and any medical information of which THE DANCE AND ART CO-OP LLC should be aware in consideration of the child's physical and mental well being.
