RULES AGREEMENT I have read and understand the Student and Dance Family Rules as outlined in the Studio and Competition Team Handbook available on www.OBXdanceacademy.com and my child and I agree to abide by them. I understand that failure to do so may result in immediate dismissal from the program with the forfeiture of all fees.
Liability Policy Dance and the required level of training and performing with which it is associated place unusual stress on the body and carry with it the risk of physical injury. On behalf of my child and myself (and if I am no longer a minor, on my own behalf), I assume the risk and agree that Outer Banks Dance Academy and the staff of these institutions shall not be liable in any way for injuries sustained during any Outer Banks Dance Academy activity. It is also understood that dance instruction involves kinetic corrections that may include physically touching a student as part of regular class work and rehearsals. Students who have concerns about this should speak with their instructors before the class begins. I assume the risk and agree that Outer Banks Dance Academy and the staff of these institutions shall not be liable in any way for injuries sustained during attendance at this Intensive or any of its related functions. As a participant or as a parent/guardian of Outer Banks Dance Academy dance programs, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume full risk of any injuries, including damages, death or loss which may be caused as a result of participation in any activities connected with Outer Banks Dance Academy. I agree to waive and relinquish all claims against Outer Banks Dance Academy and its owners, faculty, staff and consultants from all claims resulting from participation in a program. In case of accident or sickness, I consent to emergency medical care for my child/myself to be provided by ambulance or hospital personnel. Outer Banks Dance Academy is not responsible for dancer supervision outside Outer Banks Dance Academy, this includes the plaza parking lot and nearby shops.
PUBLICITY RELEASE I hereby authorize Outer Banks Dance Academy and Nick Hassell Photo to record the student's picture and voice on photographs, films, live stream, and tapes, to edit these recordings at its discretion, and to incorporate these recordings into movie and sound films on tapes, radio, and television and online broadcast programs. I also give my permission for Outer Banks Dance Academy and Nick Hassell Photo to use and license others to use these materials in any manner or media whatsoever. Outer Banks Dance Academy and Nick Hassell Photo is permitted to use these materials for any current or future publicity, advertising and sales promotion and to use the student's name, likeness and voice and biographic or other information in connection with them. I acknowledge that no promises of compensation were made by Outer Banks Dance Academy and Nick Hassell Photo for such use.
