By checking this box, you are the legal guarding of the dancer registered. You are financially responsible for all fees and tuitions on time. All fees and tuitions are non refundable.

I and my child are aware that participation in dance/acrobatics has some degree of risk/injury. All such risks are known and understood by me.

I am financially responsible for payments on this account and agree to make all payments on time. Failure to maintain a current account may result in my dancer being removed from classes and performances.

In the event of studio closure due to state mandated shut down, I understand that tuition will remain in place and classes will continue via ZOOM.

I understand that costume fees are subject to change due to material or supply cost increases.

I give permission to David De Marie Dance to use photos/videos of my child while attending classes for social media or our website.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I could be exposed to or infected from COVID-19 by attending DDD and that such exposure or infection may result in personal illness. I understand the need to evaluate myself and my child for all COVID-19 symptoms before entering the building and should any symptoms be presented I will keep my child home.
