Foundations Dance Company Liability Release Form - Assumption of Risk I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Foundations Dance Company, it’s officers, agents, employees, instructors, and all affiliated entities (hereinafter collectively referred to as FDC.) I hereby agree to release FDC Faculty and hold FDC Faculty harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. If I am a minor, my parent and / or legal guardian has also signed this document releasing FDC from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in classes at FDC and other FDC affiliated events. I warrant the below information is complete and correct. I further release FDC of all liabilities associated with my child’s attendance at any FDC classes. I acknowledge that my dancers photo may be taken while at FDC. I acknowledge that a video of my dancer may be taken while at FDC. I acknowledge that photos and videos of my dancer may be shared on FDC Social Media, website, and promotional materials.
I acknowledge that I must keep my Dance Studio Pro Account in good standing. I acknowledge that tuition is due on the 1st of every month following enrolment. I acknowledge that my account will accumulate late fees if I do not make tuition payments on time. I acknowledge that should my dancer want to withdrawal from their class, I must provide written communication via email 30 days in advance to avoid additional tuition charges, and late fees. If 30 days notice is not provided, the account will continue to be charged. I acknowledge that any fees paid to Foundations Dance Company are non-refundable. I acknowledge that I will be responsible for a one-time media fee of $25 that will allow access to the year end recital footage, that way it can be downloaded and shared. Opting out is not an option. I acknowledge that I am responsible for a costume deposit of $50, and that the remainder will be due upon costume delivery.
