I authorize Vicki Michelle (VM) to debit account each month for amount due, including additional purchases. This authorization will remain in effect until Drop Form is received on the 1st of the month and fulfilled. Additionally, I authorize VM to debit my account $10.00 for late payments, $20.00 for returned payments, $60.00 for annual registration fee, and recital fee/installments

Even with due care, there are risks associated with participation in physical activities. I release, waive, and discharge from ALL liability in event of accident/disease/illness/injury/natural disaster: Vicki Michelle, owner, employees, instructors, and other participants. VM is only responsible for your child during their scheduled class. Late pick-up is $1.00 per minute without express permission from VM Staff arranged and agreed to in advance.

I grant VM permission to use images/photos/videos of my child for marketing purposes, including social media.

- Commitment through June 2027 (or 90-day notice on 1st of the month).
- Annual registration is due upon enrollment or anniversary month.
- Dress code strictly adhered to; missing items will be billed to account.
- Tuition accepted via bank draft on or about the 1st of each month.
- Tuition holds your child’s spot. Missed classes cannot be accumulated in lieu of tuition.
- Make-up allowed in same discipline, as space allows, with good conduct. Check with front desk.
- Tardiness is disruptive and causes students to miss valuable warm-up.
- NO refund, transfer, or application of: registration, tuition, recital, or boutique items.
