CENTERSTAGE ACADEMY FOR THE ARTS, LLC
ACKNOWLEDGMENT OF RISK, RELEASE, AND PARTICIPATION AGREEMENT
PLEASE READ CAREFULLY BEFORE SIGNING. REGISTRATION IS NOT COMPLETE UNTIL THIS AGREEMENT HAS BEEN SIGNED, AND IT MUST BE COMPLETED BEFORE PARTICIPATION IN ANY CLASS OR ACTIVITY.
CenterStage Academy for the Arts, LLC, a Maryland limited liability company (“CSAA” or the “Academy”), is committed to providing a safe and supportive environment for its students. However, participation in dance, theater, movement, rehearsals, performances, workshops, camps, and related activities involves inherent risks that cannot be completely eliminated.
1. Acknowledgment and Assumption of Inherent Risks
I understand and acknowledge that participation in CSAA programs and activities involves physical activity and may include, among other things, stretching, jumping, turning, lifting, partnering, floor work, use of props or equipment, and movement on stages, dance floors, rehearsal spaces, or other surfaces.
I understand that participation may involve inherent risks of injury, including but not limited to falls, collisions, muscle strains, sprains, fractures, and other physical injuries.
I knowingly and voluntarily accept and assume the inherent risks ordinarily associated with participation in these activities, whether I am participating personally or authorizing the participation of my minor child.
2. Participant Responsibility
I understand that participants are responsible for following the instructions, safety rules, and directions of CSAA faculty and staff.
I agree that I, or my child, will not participate in an activity that is beyond the participant’s physical abilities or limitations. I agree to notify CSAA of any restrictions or limitations that may affect safe participation when appropriate.
If I or my child experiences pain, dizziness, illness, injury, or other symptoms that may affect safe participation, I understand that participation should be stopped and appropriate assistance sought.
3. Release of Claims
To the fullest extent permitted by Maryland law, I release and discharge CenterStage Academy for the Arts, LLC and its owners, members, officers, employees, instructors, contractors, guest artists, assistants, and volunteers from claims arising solely from the inherent risks ordinarily associated with voluntary participation in CSAA programs and activities.
Nothing in this Agreement is intended to release, waive, indemnify, or limit liability for negligence, wrongful acts, gross negligence, reckless conduct, intentional misconduct, or any other liability that cannot lawfully be waived or limited under Maryland law.
4. Parent or Legal Guardian Authorization
If the participant is under 18 years of age, I certify that I am the participant’s parent or legal guardian, or that I otherwise have legal authority to execute this Agreement on the participant’s behalf.
I acknowledge that I have read and understand this Agreement and voluntarily authorize my child to participate in CSAA programs and activities subject to the terms stated above.
5. Severability
If any provision of this Agreement is determined to be invalid or unenforceable, that provision shall be limited or removed only to the extent necessary, and the remaining provisions shall remain in effect to the fullest extent permitted by law.
6. Acknowledgment
By signing below, I acknowledge that:
I have carefully read this Agreement;
I understand the inherent risks associated with participation in CSAA programs and activities;
I have had the opportunity to ask questions before signing;
I am signing voluntarily; and
I understand that this Agreement affects legal rights.
