By participating in any classes, events, or activities at Elevation Academy of Dance ("EAD"), I agree to the following terms:
1. Voluntary Participation
I understand that participation in dance classes and related activities is entirely voluntary. I choose to participate—or allow my child to participate—at my own risk.
2. Assumption of Risk
I recognize that dance and physical activity involve inherent risks, including but not limited to falls, strains, sprains, and other injuries. I assume full responsibility for any and all injuries or damages that may occur as a result of participation at EAD.
3. Fitness to Participate
I affirm that I (or my child) am in good physical condition and capable of participating in dance activities. I understand that it is my responsibility to consult a physician regarding any health concerns before beginning any program.
4. Release and Waiver
In consideration of being allowed to participate, I, on behalf of myself and/or my child, release and hold harmless Elevation Academy of Dance, its owners, instructors, staff, contractors, volunteers, and affiliates from any and all liability for injuries, damages, or losses—whether to person or property—that may arise from participation in any activities on or off studio premises, including performances and travel.
5. Emergency Medical Care
In the event of an emergency, I authorize EAD staff to obtain medical treatment for myself or my child. I understand that I am financially responsible for any medical care provided.
6. Photo/Video Consent
I give permission for EAD to use photos or videos taken during classes or events for promotional purposes. (If you do not consent, please notify us in writing.)
