The Parent or Legal Guardian and the Student understand that the course includes activities that may be hazardous to the
Student, including strenuous physical activity. The Parent or Legal Guardian and the Student do hereby expressly assume
the risk of injury or harm for participating in the course and release The Rhythm Pointe, LLC, from any and all liability
or claims that the Student may have against The Rhythm Pointe, LLC with respect to any bodily injury, personal injury,
illness, death, or property damage that may result from Student’s participation in the course, whether caused by the
negligence of The Rhythm Pointe, LLC or its employees, members, managers, volunteers or agents, or otherwise.
As the Parent or Legal Guardian, I release and hold harmless The Rhythm Pointe, LLC, its owners and operators, from
any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or
injury, including death, that may be sustained by the Student and/or the undersigned, while in or upon the premises or any
premises under the control and supervision of The Rhythm Pointe, LLC, its owners and operators, or in route to or
from any of said premises.
MEDICAL EMERGENCY
The undersigned gives permission to The Rhythm Pointe, LLC, its owners and operators, to seek medical treatment for
the Student in the event they are not able to reach a Parent or Legal Guardian or designated emergency contact. The
Student does not have any physical or mental restrictions to participating fully in the program except as set forth above.
NOVEL CORONAVIRUS (COVID-19)
The novel coronavirus (COVID-19) has been declared a worldwide pandemic by the World Health Organization.
COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal,
state, and local governments and federal and state health agencies recommend social distancing and have, in many
locations, prohibited the congregation of groups of people. I understand the hazards of the novel coronavirus (COVID-
19) and am familiar with the Centers for Disease Control and guidelines regarding COVID-19. I acknowledge and
understand that the circumstances regarding COVID-19 are changing daily and that, accordingly, the CDC guidelines are
regularly modified and updated. I accept full responsibility for familiarizing myself with the most recent updates.
The Rhythm Pointe, LLC, has put in place preventative measures to reduce the spread of COVID-19; however, The
Rhythm Pointe, LLC cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further,
attendance at The Rhythm Pointe, LLC could increase your risk and your child(ren)’s risk of contracting COVID-19.
Notwithstanding the risks associated with COVID-19, which I acknowledge, I hereby willingly choose to participate in
activities at The Rhythm Pointe, LLC and release The Rhythm Pointe, LLC and its owners from any liability related to
COVID-19 which might occur as a result of participating in activities or being on the premises.
PHOTOGRAPHIC RELEASE
The Student does hereby grant and convey unto The Rhythm Pointe, LLC all rights, title, and interest in any and all
photographic images and video or audio recordings made by The Rhythm Pointe, LLC during the Student’s activities
with The Rhythm Pointe, LLC, including, but not limited to, any royalties, proceeds, or other benefits derived from such
photographs or recordings.
PAYMENT AND TUITION INFORMATION
Tuition is due by the first of each month. If accounts are not paid after the tenth of the month, there will be a $20.00 late
fee applied to the account balance.
There is a $25.00 returned check charge for any checks returned by the bank. Please review our studio policies.
The enrolling parent/guardian is the primary account holder and assumes financial responsibility. The studio does not mediate family or custody arrangements.
I have read all of the above and the Studio Policies and agree to be bound thereby.
