A CREDIT CARD NUMBER IS REQUIRED TO REGISTER (regardless of preferred payment method). Withdrawal before Sept 1st: Refund of full season, minus annual registration fee Withdrawal after Sept 1st before 1st class: Refund of Oct-Apr fees, plus costume deposit Withdrawal prior to Oct 1st: Refund of Nov-Apr fees, plus costume deposit Withdrawal prior to Nov 1st: Refund of Dec-Apr fees, plus costume deposit Withdrawal prior to Dec 1st: Refund of Jan-Apr fees, costume deposit will be forfeited Withdrawal prior to Jan 1st: Refund of Feb-Apr fees, costume deposit will be forfeited - No refund will be issued after January 1st, and Feb-Apr monthly fees will be required due to time to respace and rework choreography and routines. - Costume, competition and examination fees are all non-refundable, including cases of illness and/or injury, as per competition rules.
Physical activities such as dance, acro, drama and musical theater can carry a risk of injury to participants. I hereby release Premier Dance and its employees of any liability whatsoever arising out of any injury, damage or loss which may be sustained by myself, my minor children, or any other family members, during involvement with Premier Dance. This includes activities held in any studio, on the property at the studio location during lessons, classes, performances or rehearsals and during any other activities sponsored by Premier Dance.
In the event of an emergency, I give permission for Premier Dance staff to authorize necessary medical treatment, by a licensed physician for my child, without personal liability, if I am unable to be contacted in a timely matter.
We are asking permission to be allowed to take pictures and or videos of your children to show the community the success of our programs. Your right to privacy is taken very seriously and as such, we will use dancer names upon parent approval as well as limit any personal information being shared. These photos and videos may be used for newspaper, newsletter, display boards, website and other public information. I relinquish all rights of any photos or videos to be used by Premier Dance. By signing this release, I hereby state that I am 18 years of age or older and am the parent/legal guardian of a minor child. I hereby give permission for photographs and/or video to be taken of my children for the promotion of Premier Dance and its programs.
