I am aware that 3d takes every preventative measure towards my dancer/gymnasts safety. If for any reason my child is injured on 3d property 3d is not liable. If an incident occurs where I am unable to be reached, 3d will make their best judgment on further medical attention at my expense. If my child has a previous injury it is my responsibility to make the teacher/studio owner aware so that the injury does not progress.

**If my child drops a class, ANY PAYMENT PREVIOUSLY MADE TO 3d is NON-refundable. (tuition, costumes, competition fees etc.) If you are asked to leave the studio for any reason, your tuition, competition fees and costumes are non-refundable as well**

*I approve the use of any class pictures of my child to be used on the studios social media* IF THIS IS AN ISSUE PLEASE CONTACT 3d TO DISCUSS

*Parent meetings are not provided unless the teachers feel it is necessary. We prefer email/phone call. Thank you for understanding*

I understand that 3d is closed for classes (does not include camp) June 29-July 2. Majority of the staff is in Disney for Dance the World + it is the 4th of July holiday!

June 15-18
June 22-25
July 6-9
July 13-16
