I am aware that 3d takes every preventative measure towards my dancer/gymnastics safety. If for any reason my child is injured on 3d property 3d is not liable. If an incident occurs where I am unable to be reached, 3d will make their best judgment on further medical attention at my expense. If 3d needs to cancel class for any reason (ex. weather) a makeup class is not mandatory. 3d Dance Studio will try their hardest to schedule a makeup class for the following week, combine two classes throughout the week, or have a substitute teacher come in for the day. 3d will follow the Northampton School Districts school closings for inclement weather. I acknowledge receipt of my 3d Dance Studio LLC Swipe Card. Although the key will be in my possession I understand that it remains the property of 3d Dance Studio LLC. I agree not to release it to other persons for use. I take full responsibility for loss or damage of the swipe card during the time that it is in my possession. I am aware that I am expected to return my 3d Dance Studio LLC swipe card if I discontinue classes/are asked to leave the program. All are expected to return the card at the end of the season. If I fail to do so or I loose/damage my card I will automatically be charged $10.00 via the portal or card on file.
