I hereby grant and authorize Art To Empower, and individuals acting under its direction, the irrevocable right to photograph, videotape, record, edit, reproduce, publish, distribute, and otherwise use my image, my child's image, voice, likeness, artwork, or performance for lawful promotional, educational, fundraising, archival, and informational purposes.

These uses may include, but are not limited to:

Websites
Social media
Printed publications
Newsletters
Flyers
Brochures
Posters
Annual reports
Grant applications and reports
Press releases
Promotional videos
Educational materials
Other print and digital communications

I understand that no compensation or other consideration will be provided for these uses.

This authorization applies to all current and future media formats and shall remain in effect unless revoked in writing. Revocation will apply only to future uses and will not affect materials already produced or distributed.

I understand that all photographs, videos, recordings, and related materials become the property of Art To Empower.

I release and hold harmless Art To Empower, its officers, directors, employees, teaching artists, instructors, volunteers, contractors, and affiliated organizations from any claims arising from the use of these materials.

If the participant is under eighteen (18) years of age, this release must be signed by the participant's parent or legal guardian.
