Registration Waiver & Media Consent

By registering myself or my child with Xtreme Talent Dance Company ("XTDC"), I acknowledge and agree to the following:

Assumption of Risk & Liability Release

I understand that participation in dance classes, rehearsals, private lessons, performances, competitions, workshops, conventions, camps, and other studio activities involves inherent physical risks, including but not limited to falls, sprains, strains, fractures, concussions, illness, and other injuries.

I voluntarily assume all risks associated with participation and, to the fullest extent permitted by applicable law, release and hold harmless Xtreme Talent Dance Company, its owners, directors, instructors, choreographers, employees, assistants, volunteers, contractors, and representatives from any claims, liabilities, damages, or expenses arising from participation, except where caused by gross negligence or intentional misconduct.

Medical Authorization

I certify that I (or my child) am physically able to participate in dance activities. I agree to disclose any relevant medical conditions, injuries, allergies, or physical limitations that may affect safe participation.

In the event of a medical emergency, I authorize XTDC to obtain emergency medical treatment if I cannot be contacted. I understand that all medical expenses incurred are my sole responsibility.

Physical Instruction

I understand that dance instruction may involve appropriate hands-on guidance, spotting, or physical corrections by instructors to promote proper technique and dancer safety. These corrections will always be performed professionally and respectfully.

Photo & Video Release

I grant Xtreme Talent Dance Company permission to photograph, videotape, livestream, or otherwise record myself or my child during studio activities.

I authorize XTDC to use these images and recordings for studio-related purposes, including advertising, social media, websites, promotional materials, event programs, newsletters, and other marketing or educational publications. I understand that no compensation will be provided and that these materials become the property of XTDC.

Communication Consent

I consent to receiving communications from XTDC by email, telephone, text message, WhatsApp, or other electronic communication platforms regarding registration, scheduling, billing, studio announcements, performances, competitions, emergency notifications, and other important studio information.

Acknowledgement

By completing my registration, I acknowledge that I have read and understood this Waiver & Media Consent, voluntarily accept its terms, and certify that I am the participant or the parent/legal guardian of the participant with the authority to enter into this agreement.
