By clicking "I Agree" below I grant Dancing Arts Academy the right and permission to use and publish any photographs, videos, and electronic representations and/or sound recordings made of my child at all dance activities, including rehearsals, performances, and dance classes.

I hereby assume all responsibility for risk of damage or injury that may occur as a participant in any activity at Dancing Arts Academy. In consideration of being accepted as a student of being a parent or legal guardian of a student, the undersigned hereby releases and discharges the dance studio and agents from all claims, demands, rights or causes of action present or future, whether known, anticipated; and resulting from or arising out of or incident to, the undersigned’s use or undersigned parent/guardian’s child(ren)’s use (or intended use) of the dance studio location or facilities or equipment, or transportation or vehicles in such a place or as a result of, or incident to, engaging Dancing Arts Academy dance activities.

I have read and understand the above and hereby release Dancing Arts Academy from any and all liability.
