Enrolled minor(s) has/have permission to participate fully in all aspects of the dance program. In consideration of participation in dance programs at The Mid Shore Dance Academy, the parent(s) and/or legal guardian(s) of the enrolled minor(s) have agreed: 1) To accept all rules of The Mid-Shore Dance Academy as posted, and make payments as listed, 2) To allow photographs and video of my child(ren) during scheduled dance activities to be used for promotional purposes, 3) To monitor behavior of participants, siblings, and guests while in public areas of the studio, i.e. while waiting for class. I/We accept and assume all risks and responsibility for the losses and/or damages following any injury or disability, however caused or alleged to be caused in whole or in part by the negligence of The Mid-Shore Dance Academy, other participants, instructors, sponsors, directors, event hosts, agents, and employees.

Drop-off and Pick-up
STUDENTS UNDER THE AGE OF NINE MUST BE ACCOMPANIED BY A PARENT OR SIBLING AGED 12 OR OLDER INTO/OUT OF THE BUILDING FOR DROP-OFF AND PICK-UP!
This is for your child's safety, and is based on state law that children under the age of nine cannot be left unaccompanied without supervision. It is also the parent(s)' responsibility to make sure your child is prepared for class before you leave (hair up, shoes on, wearing proper attire).
Please do not sign up for classes that you cannot attend fully. It is disruptive to the entire class if students arrive late and/or leave early and students miss warm-up at the beginning and skill development at the end. We understand the occasional need to make an adjustment for a special event. Please just let the instructor know ahead of time and enter/exit as quietly as possible.
Please arrive BEFORE your scheduled class time to be READY on time!

Please be prompt in picking up students. Young students may get distressed if a parent is not present and teachers cannot monitor children left from a previous class when they start teaching another class.

Fee Details / Procedures:
1. At the time of registration, a non-refundable 20% deposit is paid. This reserves your space in class(es).
2. The online registration system will automatically calculate multi-class and sibling enrollment discounts.
3. The balance of the session fee will be automatically billed during the week prior to the start of the session, unless the studio has been notified of cancellation.
4. All accounts are required to register for "Auto-Pay" and have a valid payment method on file, so session balances can be processed.
5. Tuition is not discounted for classes missed due to illness, vacation, or other absences. Students can attend another class for make-up.
6. Tuition payments are non-refundable, except in the event of class cancellation by the studio.
7. Enrolled students will be automatically re-enrolled into subsequent sessions, and fees processed accordingly, unless the studio is notified by the discontinue date as listed on our calendar. This does not apply to Toddler or Summer Sessions. Re-enrollment required for these.
8. Monthly Installment Option: When enrolled for the full school year (all four 8-week sessions), monthly installment payments can be arranged instead of session fees. Full School-Year commitment required. At time of enrollment, the 20% 1st session enrollment deposit is collected automatically. Contact the studio (email us!) to set up monthly installments, and remaining 80% of 1st session, plus remaining sessions will be divided into 10 monthly payments, Sept-June.
