If any changes occur to yours or your child's personal details (eg: address, contact phone numbers, medical conditions, etc) during 2025, please notify us immediately.

Please note that the return of this form is a legal requirement to attend classes at The Dance Studio therefore it must be completed, signed and returned before your child commences classes in 2025.

If my child is involved in an emergency situation, I accept any necessary medical, hospital or ambulance service being sought and agree to pay costs arising from such action.

I accept the conditions which govern the undertaking of classes conducted by The Dance Studio Academy of Performing Arts. I have read and fully understand The Dance Studio Academy of Performing Arts Policies and Procedures AND the Indemnity Waiver Form documents as presented to me by THE DANCE STUDIO Academy of Performing Arts in the Enrolment Pack and agree to abide by all conditions set out therein.

I am aware that standard term fees are billed within the last 2 weeks of the previous term and will be direct debited via Autopay on the first day of the term. I understand that fees are billed on a full-term basis and deductions/refunds will not be given for absence from classes for reasons not specified in our payment policy.

***Summer School fees due upon registration for the program. Please note that this booking and all payments are subject to the conditions of The Dance Studio’s full Policies and Procedures. All Summer School fees paid are final and non-refundable and non-transferable under any circumstances.****

I recognise that it is impossible for staff at The Dance Studio Academy of Performing Arts to supervise my child outside of class times. For safety reasons, parents are asked to escort their children to and from the building.
