COVID – 19 WAIVER
Despite our careful attention to sterilization and disinfection, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Please know that participation in South Jersey Dance Center instructional classes and recital is at your own risk.
Although exposure is unlikely, do you accept the risk and consent to participation?
Do you agree to adhere to all of the South Jersey Dance Center safety guidelines and measures to help stop the transmission of COVID-19?
Do you agree that if someone in your household or your child is feeling under the weather and/or if you or your family have been exposed to COVID-19 that you stay home to keep all others safe?
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin HEREBY RELEASE AND HOLD HARMLESS South Jersey Dance Center their owners, employees, other participants, sponsoring agencies, sponsors, advertisers, vendors, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
SOUTH JERSEY DANCE CENTER WAIVER AND RELEASE
South Jersey Dance Center referred to as “SJDC”, desires to make its students and their parents aware of the risks associated with dance and performing arts. Students may suffer injuries, possibly minor or serious in nature while learning and developing their dance training. Parents are responsible for making their children aware of the possibility of injury and encouraging their children to follow the safety rules of the studio and instructors.
“SJDC”, its instructors, and other staff members does not accept responsibility for injuries sustained by any student during the course of class instruction or in the course of any exhibition, performance, or workshop participation, or while traveling to or from class or an affiliated event sponsored by South Jersey Dance Center.
With the above in mind, and being fully aware of the risks and possibility of injury involved, I hereby affirm that I, and/or my child/children are in good physical condition and do not suffer from any disability that would prevent or limit participation in any program offered by “SJDC”. I, my executors or other representatives, waive and release “SJDC”, its director, Shaelyn Shire, and its employees or representatives whether paid or volunteer, from any claims, demands, and causes of action arising from my or my child/children’s participation in any program offered or sponsored by “SJDC”. I also hereby release “SJDC”, its director, Shaelyn Shire, and its employees or representatives whether paid or volunteer, from any liability now or in the future including but not limited to muscle strain, pulls, tears, broken bones, shin splints, heat prostration, knee or lower back or foot injuries, and any other illness, soreness or injury, however caused, occurring before, during, or after participation in any programs offered at “SJDC”, or at any time, while in the vicinity of the premises of the above stated business, or in any activity sponsored, represented or organized by “SJDC”, its director, Shaelyn Shire, or any employees or representatives, whether paid or volunteer, for whatever reason.
I also affirm that I now have and will continue to provide proper hospitalization, health, and accident insurance coverage adequate for both my child’s protection and my own protection. I understand that “SJDC” does not carry accident insurance and I still want to enroll myself, or my child/children in classes at “SJDC”.
By signing, I hereby affirm that I have read and fully understand and agree with the above waiver. I also affirm that I have read and fully understand and agree with the studio policies, and that I am responsible for payment of all “SJDC” related fees.
RELEASE FOR USE OF PHOTOGRAPHS/VIDEO
I hereby give South Jersey Dance Center permission to take, use, re-use, publish, or re-publish photographs and/or videos,
in whole or in part, individually or in conjunction with others, in any medium including (but not limited to) display, promotion, and/or advertising. I also release South Jersey Dance Center from any and all claims and demands arising from or in connection with the use of my photographs, or the photographs of my child/children.
I have read and fully understand the intent and purpose of this release and am signing it without reservation.
