Phoenix Stage School – Waiver & Policy Agreement

MEDIA CONSENT
From time to time, photographs and videos of classes or performances may be sent out for rehearsal purposes or may appear on the school website, social media, or other promotional materials. By ticking this box, you are giving consent for your child to be included in such media. If you do not wish for your child to appear in photos or videos, please contact us. Please note that this would mean your child would be unable to participate in any performances (including the summer show), as these events are recorded or photographed. In such cases, it is the parent’s responsibility to explain to the child that they will need to be repositioned whenever photographs are being taken.

HEALTH & SAFETY
All classes are taught with safety as the highest priority. However, as with all performance or sporting activities, there is always a small risk of injury. Phoenix Stage School, and any venues used for classes or performances, cannot be held responsible for such injuries. We strongly recommend that students wear jazz shoes during dance classes to help prevent injury. It is the student’s responsibility to ensure they are wearing the appropriate footwear, as staff will not be monitoring this.

ATTENDANCE POLICY
Regular attendance is essential to ensure students can fully participate and progress in their classes. In the lead-up to shows, competitions, or performances, missing multiple classes may result in a student being removed from certain roles or parts at the school’s discretion.
