Dance is a physical activity, which can involve risk of personal injury. Each student and guardian is responsible for ensuring that they are physically and mentally fit for dance class. You are responsible for informing instructor(s) of any physical limitation(s) which may prevent full participation in class. High Definition Dance reserves the right to deny participation or enrollment if High Definition Dance determines it is in its sole discretion that a student represents a danger to themselves or others. All students must be covered by a health insurance policy at all times during participation in activities at High Definition Dance. If injury occurs, you understand that your health insurance policy is the only source of payment for your medical care.
By signing your signature on your paper form, as well as by checking the box here online, you agree to waive, release, discharge and agree to hold harmless High Definition Dance and any of its parent, subsidiaries, affiliates or subcontractors, owners and employees for any liability, claims, or demands for personal injuries, sickness, or death, as well as for expenses due to loss or damage to personal property or lost wages in all cases other than the gross negligence or willful misconduct of High Definition Dance.
