Terms & Conditions Of Enrolment - Suzie Ryrie School of Dance (SRSOD) 1. Authorisation for Medical Attention In case of an accident or other emergency resulting in the need for immediate medical attention I hereby give permission for SRSOD staff to administer first aid. I hereby give permission for the student to be transported by ambulance to hospital. I accept all expenses incurred. 2. Accident / Injury Disclaimer I understand that participating in dance classes has risks of injury. I understand that whilst SRSOD offer safe dance practices, injuries may sometimes occur and students dance at my own risk. I do not hold SRSOD responsible for any injuries. 3. Permission for Photos / Recordings / Publicity I understand that SRSOD events may be filmed and/or photographed and are copyrighted to SRSOD, the Recording company and the Photographer. I hereby give my consent and/or permission for any photos and/or recordings of my child whilst at SRSOD studios and/or events to be used for publicity purposes. 4. Payment of Accounts I understand that all tuition, fees and levies are non-refundable and non-transferrable except where indicated. Payment methods and the fees associated with each method will be available on request from SRSOD Administration. I understand that the 5% early payment discount expires on the day before classes start each term and will be shown on the tuition statement. I understand that if I do not receive a statement for any reason it is my responsibility to seek out the amount due for the purposes of either early or on time payment. I understand I may be charged a fee if payments are not received by the due date and agree to pay such fee. I understand that my child may be excluded from classes if fees are not paid when due. I understand that all costumes are provided on a hire basis and remain the property of SRSOD. I understand and agree to pay any associated fees incurred by a debt collector. 5. Exclusion SRSOD reserves the right to exclude any student temporarily or permanently, if this action is in the interests of students or SRSOD. This right extends to non-payment of fees and breaching any of the Terms & Conditions of Enrolment 6. Intellectual Property I hereby acknowledge that all choreography and costumes for classes, performances and events of SRSOD are and remain the property of SRSOD and may not be used without written permission of SRSOD. 7. Personal Property SRSOD takes no responsibility for any personal property left or stored on the premises. 8. Code Of Conduct I understand that all concerns, problems and/or complaints must be made in writing and emailed to info@suzieryrie.com.au. I understand that bullying will not be tolerated at SRSOD whether in the studio, at events or online. I understand that SRSOD staff have trained in and have adopted the Child Safe Policy and are committed to upholding the policies outlined in the current legislation.
