Terms & Conditions Of Enrolment - Suzie Ryrie School of Dance
1 Authorisation for Medical Attention: In case of an accident or other emergency resulting in the need for immediate medical attention I hereby give permission for Suzie Ryrie School of Dance to administer first aid. I hereby give permission for the student to be transported by ambulance to hospital. I accept all expenses incurred.
2 Accident / Injury / Disclaimer: I understand that participating in dance classes has risks of injury. I understand that whilst Suzie Ryrie School of Dance offer safe dance practices, injuries may sometimes occur and you dance at your own risk. I do not hold Suzie Ryrie School of Dance responsible for any injuries.
3 Permission for Photo/ DVD/ Publicity: I understand all of Suzie Ryrie School of Dance’s events are filmed and photographed and are copyrighted to Suzie Ryrie School of Dance, the DVD company and the Photographer. I hereby give my consent, permission for any photos and videos taken of my child whilst at Suzie Ryrie School of Dance studio or related events to be used for publicity purposes.
4 Fee Accounts Payment of accounts: I understand that all fees and levies are Non Refundable, Non Transferrable.
Payment will only be accepted via EFT, Credit Card (1% surcharge) cash or internet transfer. Cheques will not be accepted.
I understand that the 5% early payment discount expires on the nominated date on the invoice.
I understand that my child may be refused from classes if fees are not paid when due.
I understand that all costumes are provided on a hire basis and remain the property of Suzie Ryrie School of Dance.
I understand and agree to pay all Fees etc in full when due, as listed above. ( Note: payment plans are available)
I understand and agree to pay a 10% or $15 ( whichever is the greater amount) administration fee if term fees are in arrears, if not on a payment plan.
I understand and agree to pay any associated fees incurred by a debt collector.
Any changes to the Terms & Conditions Of Enrolment Suzie Ryrie School of Dance must be requested for in writing for prior approval by Suzie Ryrie School of Dance
Exclusion: Suzie Ryrie School of Dance reserves the right to exclude any student temporarily or permanently, if this action is in the interest of the student or Suzie Ryrie School of Dance. This right extends to non-payment of fees and breeching any of the Terms & Conditions of Enrolment

Intellectual Property: I hereby acknowledge that all Choreography and Costumes for performances and alike taught at Suzie Ryrie School of Dance are and remain the property of Suzie Ryrie School of Dance and may not be used without written permission of Suzie Ryrie School of Dance.
Personal Property: Suzie Ryrie School of Dance takes no responsibility for any personal property left or stored on the premises.
Code Of Conduct: I understand that all concerns, problems and/or complaints must be done in writing and sent to Suzie Ryrie School of Dance email info@suzieryrie.com.au . I understand that bullying will not be tolerated at Suzie Ryrie School of Dance whether in the studio, at events or online.
I understand that SRSOD Staff have trained in and have adopted the child safe policy and are committed to upholding the policies outlined in the current legislation.
